Overview of the medicine
Penomer (Meropenem) is a broad-spectrum carbapenem antibiotic used to treat a wide range of bacterial infections. It is administered intravenously and is effective against many Gram-positive and Gram-negative bacteria, including anaerobic strains. It is typically reserved for serious or complicated infections.
Uses & Indications
Dosage
Adults
For most infections: 500 mg to 1 g intravenously every 8 hours, depending on the severity and type of infection. For bacterial meningitis or severe infections: 2 g intravenously every 8 hours.
Elderly
Dosage adjustment may be necessary based on renal function. No specific dose adjustment solely based on age for normal renal function.
Renal_impairment
Requires significant dose reduction based on creatinine clearance (e.g., CrCl 26-50 mL/min: 1 g every 12 hours; CrCl 10-25 mL/min: 500 mg every 12 hours; CrCl <10 mL/min: 500 mg every 24 hours). Hemodialysis patients require a supplementary dose after each dialysis session.
How to Take
Administer by intravenous infusion over 15 to 30 minutes, or by intravenous bolus injection over 3 to 5 minutes for doses up to 1 gram. It should be reconstituted with an appropriate sterile diluent.
Mechanism of Action
Meropenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This leads to the disruption of bacterial cell wall formation and ultimately cell lysis and death.

Primarily excreted unchanged by the kidneys (approximately 70% as unchanged drug within 12 hours).

Undergoes limited metabolism; primarily hydrolyzed by dehydropeptidase-I (which is inhibited by cilastatin, though meropenem is stable to DHP-I).
Side Effects
Contraindications
- •Known hypersensitivity to meropenem or to any other carbapenem antibiotic.
- •Severe hypersensitivity to any other beta-lactam antibacterial agent (e.g., penicillins, cephalosporins).
Drug Interactions
Probenecid
Probenecid competes with meropenem for active tubular secretion, leading to increased plasma concentrations and prolonged half-life of meropenem. Co-administration should be avoided unless the benefit outweighs the risk.
Valproic acid
Concomitant administration with meropenem may reduce valproic acid plasma concentrations below the therapeutic range, leading to an increased risk of seizures. Co-administration is generally not recommended.
Storage
Store below 30°C, protect from light. Do not freeze. Reconstituted solution should be used immediately or stored as per specific instructions (e.g., for a limited time in a refrigerator).
Overdose
In the event of an overdose, discontinue meropenem and institute general supportive treatment. Meropenem can be removed by hemodialysis.
Pregnancy & Lactation
Pregnancy: Use only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Lactation: Meropenem is excreted into human breast milk. Caution should be exercised when administered to a nursing mother.
