Peritop (Cyproheptadine) is an antihistamine used to relieve allergy symptoms such as watery eyes, runny nose, itching eyes/nose, sneezing, and hives. It can also be used to stimulate appetite.
Uses & Indications
Dosage
Adults
4 mg three times a day initially; dosage may be adjusted according to response, usually 4-20 mg daily. Max: 32 mg/day.
Elderly
Lower doses recommended; monitor closely for sedation and anticholinergic effects. Initial dose may be 4 mg once or twice daily.
Renal_impairment
Use with caution; dose adjustment may be necessary due to potential for accumulation of metabolites.
How to Take
Administer orally with or without food. Take the prescribed dose as directed by a physician. Liquid forms should be measured with a special spoon or device.
Mechanism of Action
Cyproheptadine acts as a potent histamine H1 receptor antagonist, blocking the effects of histamine and thereby alleviating allergy symptoms. It also has serotonin antagonist properties and weak anticholinergic effects. Its appetite-stimulating effect is thought to be related to its serotonin antagonism at the hypothalamus.

Side Effects
Contraindications

Drug Interactions
MAO Inhibitors
May prolong and intensify anticholinergic effects of antihistamines; concomitant use is contraindicated.
CNS Depressants
Increased sedation and CNS depression (e.g., alcohol, sedatives, hypnotics, tranquilizers, opioids)
Anticholinergic Drugs
Increased risk of anticholinergic side effects (e.g., dry mouth, urinary retention, blurred vision).
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children. Do not freeze syrup.
Overdose
Symptoms of overdose may include severe drowsiness, dizziness, dilated pupils, dry mouth, flushed skin, fever, hallucinations, ataxia, or seizures. Management is supportive and symptomatic, including gastric lavage or activated charcoal if recent ingestion, and maintaining respiration and circulation. Physostigmine may be considered in severe anticholinergic toxicity.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Not recommended during lactation as it may pass into breast milk and potentially harm a nursing infant or cause sedation.
