Description
Overview of the medicine
Perol is a novel non-opioid analgesic and anti-inflammatory agent designed to alleviate mild to moderate pain and reduce inflammation. Its precise mechanism of action is distinct from traditional NSAIDs, offering a potentially improved side effect profile.
Uses & Indications
Dosage
Adults
Oral, 20 mg every 8 hours as needed. Maximum 60 mg per day.
Elderly
No specific dosage adjustment required, but use with caution due to potential decreased renal function. Start with the lowest effective dose.
Renal_impairment
Mild to moderate impairment: Reduce dose to 10 mg every 12 hours. Severe impairment: Not recommended.
How to Take
Perol tablets should be swallowed whole with water, preferably with food or milk to minimize gastrointestinal discomfort.
Mechanism of Action
Perol is hypothesized to selectively modulate pain pathways in the central nervous system and inhibit certain inflammatory mediators, possibly through a novel receptor interaction or enzyme inhibition pathway yet to be fully elucidated. It is not an opioid and does not act via COX inhibition.
Pharmacokinetics
Onset
30-60 minutes
Excretion
Mainly excreted renally as metabolites, with a small percentage excreted unchanged.
Half life
Approximately 4-6 hours
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily hepatic metabolism via cytochrome P450 enzymes (specific isoenzymes to be determined).
Side Effects
Contraindications
- Hypersensitivity to perol or any excipients
- Severe hepatic impairment
- Severe renal impairment
- Active gastrointestinal bleeding or peptic ulcer disease
Drug Interactions
Lithium
May increase lithium plasma concentrations. Monitor lithium levels.
Warfarin
May increase anticoagulant effect. Monitor INR closely.
Diuretics
May reduce the natriuretic effect of diuretics.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of overdose, seek immediate medical attention. Symptoms may include nausea, vomiting, abdominal pain, drowsiness, and headache. Management is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding due to lack of data on excretion into breast milk.

Lab Monitoring
- Periodic monitoring of renal function (creatinine, BUN) for long-term use or in patients with impaired renal function.
- Liver function tests (ALT, AST) periodically in patients with hepatic impairment or on long-term therapy.
Doctor Notes
- Advise patients on proper dosage and administration.
- Monitor for signs of hypersensitivity reactions.
- Consider alternative therapy if no significant pain relief is achieved after a reasonable trial period.
Patient Guidelines
- Take Perol exactly as prescribed by your doctor.
- Do not exceed the recommended dose.
- Inform your doctor if you experience any unusual side effects.
Missed Dose Advice
