Description
Overview of the medicine
Picorif 10 mg Tablet contains Sodium Picosulfate, a stimulant laxative used for the short-term treatment of constipation and for bowel preparation before medical procedures.
Uses & Indications
Dosage
Adults
10-20 mg (1-2 tablets) once daily at night. Adjust dose according to individual response.
Elderly
Start with the lowest effective dose (e.g., 5 mg) and adjust as needed.
Renal_impairment
No specific dose adjustment required for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Swallow the tablet whole with water, preferably at night to produce a bowel movement the next morning.
Mechanism of Action
Sodium Picosulfate is a prodrug that is converted by colonic bacteria into its active metabolite, bis-(p-hydroxyphenyl)-pyridyl-2-methane (BHPM). BHPM acts directly on the colonic mucosa, stimulating peristalsis and promoting fluid and electrolyte accumulation in the colon, leading to increased stool bulk and easier passage.
Pharmacokinetics
Onset
6-12 hours
Excretion
Mainly excreted via feces as active metabolite; some urinary excretion of conjugated metabolites.
Half life
Not clearly defined for the prodrug, active metabolite has a short half-life.
Absorption
Minimal absorption from the gastrointestinal tract; primarily converted by colonic bacteria.
Metabolism
Converted to active metabolite (BHPM) by colonic bacteria; subsequent hepatic conjugation and enterohepatic recirculation for a small fraction.
Side Effects
Contraindications
- Hypersensitivity to sodium picosulfate or any component of the tablet
- Acute surgical abdomen, severe abdominal pain with nausea and vomiting, intestinal obstruction, inflammatory bowel disease, severe dehydration
Drug Interactions
Antibiotics (broad-spectrum)
May reduce laxative effect by altering colonic flora responsible for prodrug conversion.
Diuretics, Adrenocorticosteroids
May increase electrolyte imbalance, especially hypokalemia.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe abdominal cramps, fluid and electrolyte disturbances (especially hypokalemia), and severe diarrhea. Management involves supportive care, correction of fluid and electrolyte imbalances.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the risks. Limited data available on use during lactation; caution advised.
