Overview of the medicine
Piglit is an oral antidiabetic agent used to improve glycemic control in adults with type 2 diabetes mellitus. It belongs to the thiazolidinedione class of drugs.
Uses & Indications
Dosage
Adults
Initial 15 mg or 30 mg once daily. Max 45 mg once daily.
Elderly
No specific dose adjustment, but start at lower end of dosage range.
Renal_impairment
No dose adjustment needed for renal impairment.
How to Take
Orally, once daily, with or without food.
Mechanism of Action
Piglit is a potent and highly selective agonist for peroxisome proliferator-activated receptor-gamma (PPARγ). PPARγ receptors are found in adipose tissue, skeletal muscle, and the liver. Activation of PPARγ leads to increased insulin sensitivity in these tissues, reducing insulin resistance.

Onset
Glycemic control begins within 2 weeks, full effect within 2-3 months.
Excretion
Primarily via bile into feces (55%) and urine (45%) as metabolites.
Half life
Approximately 16-24 hours for parent drug and active metabolites.
Absorption
Rapidly absorbed. Peak plasma concentration (Tmax) in 2-4 hours. Bioavailability 80-90%.
Metabolism
Extensively metabolized in the liver by CYP2C8 and CYP3A4 to active and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to piglit or any component of the formulation
- New York Heart Association (NYHA) Class III or IV heart failure
- Diabetic ketoacidosis
- Active bladder cancer
Drug Interactions
Insulin
Increased risk of hypoglycemia and fluid retention when used together. Closely monitor glucose and signs of fluid retention.
Rifampin
Strong CYP2C8 inducer; significantly decreases piglit levels. Co-administration may require increased piglit dose or alternative therapy.
Gemfibrozil
Potent CYP2C8 inhibitor; significantly increases piglit levels. Co-administration requires dose reduction of piglit or alternative therapy.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to fluid retention and hypoglycemia (if co-administered with insulin or sulfonylurea). Management is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during breastfeeding due to potential risk to infant.
