Description
Overview of the medicine
Pilosol 4-2 Rectal Ointment is a combination medication containing a local anesthetic (Lidocaine) and a corticosteroid (Hydrocortisone), used for the symptomatic relief of pain, itching, swelling, and discomfort associated with hemorrhoids and other anorectal disorders.
Uses & Indications
Dosage
Adults
Apply a small amount to the affected area up to 2-3 times daily, or as directed by a physician. For internal hemorrhoids, use the applicator provided.
Elderly
Same as adult dosage, but caution should be exercised. Start with lower frequency if skin atrophy is a concern.
Renal_impairment
No specific dosage adjustment required for topical use, but prolonged use in extensive areas should be monitored due to potential systemic absorption.
How to Take
For external and intra-rectal use only. Wash hands before and after application. Cleanse the affected area gently before applying the ointment. For internal use, attach the applicator to the tube, insert gently into the rectum, and squeeze the tube.
Mechanism of Action
Lidocaine blocks nerve impulse conduction, providing local anesthetic effect. Hydrocortisone is a corticosteroid that reduces inflammation, itching, and swelling by inhibiting various inflammatory mediators.
Pharmacokinetics
Onset
Within minutes (Lidocaine for anesthesia), hours to days (Hydrocortisone for full anti-inflammatory effect).
Excretion
Renal excretion of metabolites.
Half life
Lidocaine: ~1.5-2 hours (systemic); Hydrocortisone: ~1.5-2 hours (systemic, but local effect is prolonged).
Absorption
Minimal systemic absorption through intact skin/mucosa; however, absorption increases with compromised skin or prolonged use.
Metabolism
Primarily hepatic (Lidocaine via CYP450, Hydrocortisone primarily by reduction).
Side Effects
Contraindications
- •Hypersensitivity to Lidocaine, Hydrocortisone, or any other component of the formulation.
- •Viral, fungal, or tubercular lesions of the skin in the treatment area.
- •Perianal or genital pruritus without inflammation.
Drug Interactions
General
Systemic absorption is minimal, making significant drug interactions unlikely. However, caution with other local anesthetics or corticosteroids if used concomitantly over large areas.
Storage
Store below 30°C in a dry place, protected from light. Do not freeze.
Overdose
Systemic overdose is unlikely with topical application. However, excessive or prolonged use, especially over large, broken skin areas, could lead to systemic effects of lidocaine (CNS depression, cardiovascular effects) or hydrocortisone (Cushing's syndrome, adrenal suppression). Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use with caution during pregnancy and lactation, only if clearly needed and after consulting a physician. Minimal systemic absorption reduces risk, but potential effects on the fetus/infant should be weighed against benefits.
