Overview of the medicine
Pizofen-TS 15 mg Tablet contains Pizotifen, which is primarily used for the prophylactic treatment of migraine and cluster headaches. It also has appetite-stimulating properties and may be used to increase body weight in underweight individuals.
Uses & Indications
Dosage
Adults
For migraine prophylaxis, 1 tablet (15 mg) once daily, preferably at night. The dose may be adjusted based on individual response and tolerability, up to a maximum recommended dose by physician. For appetite stimulation, dosage should be individualized.
Elderly
Initiate treatment with a lower dose, e.g., 7.5 mg (if a divisible tablet is available) or as prescribed by a physician, due to potential increased sensitivity and reduced drug clearance. Monitor closely for adverse effects.
Renal_impairment
Use with caution in patients with renal impairment. Dose adjustment may be necessary; consult a physician for specific guidance based on severity of impairment.
How to Take
Take the tablet orally, with or without food. It is generally recommended to take it at night due to its sedative effects.
Mechanism of Action
Pizotifen acts primarily as a serotonin (5-HT1 and 5-HT2) antagonist, stabilizing cranial blood vessels and preventing migraine onset. It also exhibits antihistaminic (H1) and mild anticholinergic properties. Its appetite-stimulating effect is believed to be mediated through antagonism of 5-HT2C receptors.
Pharmacokinetics
Onset
Prophylactic effect for migraine may take several weeks to manifest. Not for acute migraine relief.
Excretion
Approximately two-thirds of the dose is excreted in the urine, predominantly as metabolites, with a small portion excreted in feces.
Half life
Approximately 18-26 hours (terminal half-life).
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, reaching peak plasma concentrations within 5-8 hours.
Metabolism
Extensively metabolized in the liver, primarily by glucuronidation, to an N-glucuronide conjugate which is the main metabolite.
Side Effects
Contraindications

Drug Interactions
Anticholinergic Agents
May potentiate anticholinergic side effects (e.g., dry mouth, blurred vision, urinary retention).
CNS Depressants (e.g., alcohol, sedatives, hypnotics)
May enhance the sedative effects of Pizotifen.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, dizziness, confusion, disorientation, tachycardia, hypotension, and respiratory depression. Treatment is symptomatic and supportive. Gastric lavage or activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pizotifen is excreted in breast milk; therefore, it is not recommended during lactation.
