Description
Overview of the medicine
Pneumovax 23 is a vaccine used to prevent serious infections caused by 23 types of pneumococcal bacteria. These infections can include pneumonia, meningitis, and bloodstream infections.
Uses & Indications
Dosage
Adults
A single 0.5 mL dose administered intramuscularly (IM) or subcutaneously (SC). Re-vaccination may be considered after 5 years for certain high-risk individuals.
Elderly
A single 0.5 mL dose administered intramuscularly (IM) or subcutaneously (SC). Generally, one dose is sufficient, but re-vaccination may be considered based on individual risk factors and guidelines.
Renal_impairment
No specific dose adjustment is needed for individuals with renal impairment.
How to Take
Administer as a single 0.5 mL dose intramuscularly (IM) or subcutaneously (SC), typically into the deltoid muscle or lateral thigh. Do not inject intravenously.
Mechanism of Action
The vaccine contains purified capsular polysaccharides from 23 specific serotypes of Streptococcus pneumoniae. Upon administration, it stimulates the immune system to produce type-specific antibodies, which provide protection against invasive pneumococcal disease caused by these serotypes.
Pharmacokinetics
Onset
Immune response (antibody production) typically begins within 2-3 weeks post-vaccination.
Excretion
Not applicable to the vaccine antigen; antibodies are cleared over time through normal physiological processes.
Half life
Not applicable to the vaccine antigen itself; refers to the persistence of vaccine-induced antibodies, which varies.
Absorption
As a vaccine, traditional absorption kinetics do not apply. The antigens are processed by antigen-presenting cells to elicit an immune response.
Metabolism
Antigens are processed and presented by immune cells; they are not metabolized in the same way as small molecule drugs.
Side Effects
Contraindications
- •Hypersensitivity to any component of the vaccine, including any excipients.
- •Acute severe febrile illness (vaccination should be postponed until the patient has recovered).
Drug Interactions
Other vaccines
Pneumovax 23 can be administered concurrently with other vaccines (e.g., influenza vaccine), but separate injection sites and different syringes should be used. Consult guidelines for specific co-administration recommendations.
Immunosuppressive therapy
Immunosuppressive treatments (e.g., corticosteroids, chemotherapy) may reduce the immune response to the vaccine, potentially leading to lower antibody levels and reduced protection.
Storage
Store refrigerated at 2°C to 8°C (36°F to 46°F). Do not freeze. Discard if vaccine has been frozen. Protect from light.
Overdose
Overdose with a single-dose vaccine like Pneumovax 23 is unlikely. In the event of an overdose, treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. It is unknown if vaccine antigens are excreted in human milk; caution should be exercised when administered to a nursing woman. Consult a physician.
