Overview of the medicine
Podoxi (Cefpodoxime Proxetil) is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including respiratory tract infections, urinary tract infections, skin infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
Acute exacerbations of chronic bronchitis: 200 mg every 12 hours for 10 days. Community-acquired pneumonia: 200 mg every 12 hours for 14 days. Pharyngitis/tonsillitis: 100 mg every 12 hours for 5-10 days. Uncomplicated UTI: 100 mg every 12 hours for 7 days.
Elderly
No specific dose adjustment required unless renal function is impaired.
Renal_impairment
Creatinine clearance 30-39 mL/min: Administer usual dose every 24 hours. Creatinine clearance <29 mL/min: Administer usual dose every 48 hours.
How to Take
May be taken with food to enhance absorption. Take the full course of medicine as prescribed, even if symptoms improve.
Mechanism of Action
Cefpodoxime Proxetil is a prodrug that is de-esterified in vivo to Cefpodoxime. Cefpodoxime is a bactericidal agent that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), which interferes with the peptidoglycan synthesis necessary for the integrity and rigidity of the bacterial cell wall, leading to cell lysis and death.

Contraindications

Drug Interactions
Probenecid
Increases plasma concentrations of cefpodoxime by decreasing its renal excretion.
Nephrotoxic drugs (e.g., aminoglycosides)
Monitor renal function carefully if co-administered, as risk of nephrotoxicity may increase.
Antacids/H2 blockers/Proton pump inhibitors
May decrease absorption of cefpodoxime, leading to reduced efficacy. Administer cefpodoxime 2-3 hours before or after these agents.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2-8°C) and used within 14 days.
Overdose
Symptoms of overdose include nausea, vomiting, epigastric distress, and diarrhea. In severe cases, encephalopathy may occur. Treatment is supportive; hemodialysis may be useful in reducing plasma levels.
