Description
Overview of the medicine
Propofol is an intravenous general anesthetic used for the induction and maintenance of general anesthesia. It is also used for sedation of mechanically ventilated patients in intensive care units (ICU) and for conscious sedation during diagnostic and surgical procedures.
Uses & Indications
Dosage
Adults
Induction of Anesthesia: 1.5-2.5 mg/kg IV. Maintenance of Anesthesia: 4-12 mg/kg/hr infusion. ICU Sedation: 0.3-4.0 mg/kg/hr.
Elderly
Reduced doses recommended due to potential for greater cardiorespiratory depression. Titrate slowly to effect.
Renal_impairment
No specific dose adjustment is generally required, but caution is advised in severe renal impairment.
How to Take
For intravenous administration only. Should be administered by trained personnel in a hospital or clinic setting. Shake well before use. May be diluted with 5% Dextrose Injection.
Mechanism of Action
Propofol produces its anesthetic and sedative effects by enhancing the activity of gamma-aminobutyric acid (GABA) at the GABA-A receptor, leading to central nervous system depression.
Pharmacokinetics
Onset
9-30 seconds
Excretion
Primarily renal, as metabolites.
Half life
Elimination half-life: 2-4 hours; context-sensitive half-time is longer with prolonged infusions.
Absorption
Rapid and complete distribution from the central compartment to peripheral tissues.
Metabolism
Primarily hepatic, via conjugation to glucuronides and oxidation.
Side Effects
Contraindications
- Hypersensitivity to propofol, soy, eggs, or any component of the formulation.
- Pediatric patients for sedation in intensive care units (risk of Propofol Infusion Syndrome).
Drug Interactions
Opioids
May potentiate the sedative and respiratory depressant effects of propofol.
Benzodiazepines
Increased sedative and hypnotic effects.
Muscle Relaxants
May enhance neuromuscular blockade.
Volatile Anesthetics
Synergistic effects; may require lower doses of propofol.
Storage
Store below 25-30°C. Do not freeze. Protect from light. Discard any unused portion.
Overdose
Overdose can lead to cardiorespiratory depression, hypotension, and apnea. Management involves immediate discontinuation, supportive measures like mechanical ventilation, and administration of vasopressors for hypotension.
Pregnancy & Lactation
Pregnancy Category B/C. Use with caution in pregnant women; crosses the placenta. Excreted into breast milk, caution advised in nursing mothers.

Lab Monitoring
- Continuous monitoring of vital signs (blood pressure, heart rate, oxygen saturation, respiratory rate)
- For prolonged ICU sedation: Triglyceride levels, Creatine Phosphokinase (CPK), acid-base status
Doctor Notes
- Administer only in settings equipped for resuscitation, by personnel trained in general anesthesia and airway management.
- Monitor vital signs continuously, especially respiratory and cardiovascular function.
- Be aware of the risk of PRIS with prolonged infusions in ICU patients; monitor for metabolic acidosis, rhabdomyolysis, and cardiac failure.
Patient Guidelines
- Patients will be unconscious during administration, so information is for caregivers/family.
- Discuss the procedure and potential risks with the medical team.
- Follow post-procedure instructions carefully, especially regarding activity and diet.
Missed Dose Advice
Not applicable as it is administered by medical professionals in a controlled setting.
Driving Precautions
Do not drive or operate machinery for at least 24 hours after receiving Pofol due to potential residual sedation and impaired cognitive function.
Lifestyle Advice
- Avoid driving or operating heavy machinery for at least 24 hours after administration.
- Avoid alcohol and other CNS depressants for at least 24 hours post-procedure.
