Description
Overview of the medicine
Povidone-iodine 10% w/v solution is a broad-spectrum topical antiseptic used for the prevention and treatment of skin infections, wound disinfection, and pre-operative skin preparation.
Uses & Indications
Dosage
Adults
Apply undiluted (or appropriately diluted as per specific indication) to the affected area 1-3 times daily or as directed by healthcare professional. For pre-operative skin preparation, apply liberally and allow to dry before surgery.
Elderly
No specific dosage adjustment needed, but use with caution in elderly patients, especially those with pre-existing thyroid disorders, due to potential iodine absorption.
Renal_impairment
Use with caution in patients with severe renal impairment due to potential for systemic iodine absorption and accumulation.
How to Take
For external use only. Apply directly to the affected skin area. May be covered with a sterile dressing if needed. Do not swallow. Avoid contact with eyes and inner ear.
Mechanism of Action
Povidone-iodine exerts its antiseptic action by slowly releasing free iodine when in contact with skin or mucous membranes. Free iodine oxidizes proteins and nucleic acids of microorganisms, disrupting their cell structure and metabolic processes, leading to rapid cell death. It is effective against a broad range of bacteria, viruses, fungi, protozoa, and spores.
Pharmacokinetics
Onset
Rapid (within 30 seconds to minutes)
Excretion
Primarily renal excretion of iodide.
Half life
Variable, for absorbed iodine, it can be several hours to days, depending on thyroid function and renal clearance.
Absorption
Systemic absorption of iodine from topical povidone-iodine is minimal from intact skin, but can increase with large areas of application, damaged skin, or mucous membranes. Absorbed iodine is excreted via the kidneys.
Metabolism
Absorbed iodide is handled by the body similar to natural iodide, largely taken up by the thyroid gland for hormone synthesis or excreted.
Side Effects
Contraindications
- Known hypersensitivity to iodine or povidone
- Thyroid disorders (hyperthyroidism, goiter)
- In neonates and infants (especially premature)
- Before and after radioiodine scintigraphy or therapy
Drug Interactions
Lithium
Concurrent use with lithium may lead to additive hypothyroid effects.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight. Keep out of reach of children.
Overdose
Systemic overdose from topical application is unlikely but could lead to iodism (metallic taste, increased salivation, sore throat, etc.) or thyroid dysfunction, especially with prolonged use over large areas of damaged skin. Management involves supportive care and discontinuation of the product.
Pregnancy & Lactation
Use with caution during pregnancy and lactation. Excessive iodine absorption can affect fetal thyroid function or be excreted in breast milk. Consult a doctor before use.
