Description
Overview of the medicine
Povidone-iodine is a broad-spectrum antiseptic used for skin disinfection, wound care, and infection prevention. It is effective against bacteria, fungi, viruses, and protozoa.
Uses & Indications
Dosage
Adults
Apply directly to the affected skin area 1-3 times daily, after cleaning the area. May be covered with a sterile dressing if needed.
Elderly
No specific dosage adjustment required. Use with caution in those with pre-existing thyroid conditions.
Renal_impairment
Use with caution, especially on large areas of broken skin, due to the potential for systemic iodine absorption and exacerbation of renal impairment.
How to Take
For external use only. Clean the affected area thoroughly before applying a thin layer of ointment. Do not swallow. Avoid contact with eyes and mucous membranes.
Mechanism of Action
Povidone-iodine releases free iodine when it comes into contact with the skin or mucous membranes. The iodine then oxidizes microbial cellular components, leading to cell death. It has a rapid and sustained bactericidal action.
Pharmacokinetics
Onset
Rapid onset of antimicrobial action, typically within 15-30 seconds.
Excretion
Primarily via renal excretion if absorbed systemically.
Half life
Not well-defined for topical application due to minimal systemic absorption. Systemically absorbed iodine has a short half-life, but prolonged exposure can lead to accumulation.
Absorption
Minimal systemic absorption occurs through intact skin. Absorption may be increased on broken or burnt skin, leading to potential systemic effects of iodine.
Metabolism
Systemically absorbed iodine is metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to iodine, povidone, or any excipients
- History of thyroid disorders (e.g., hyperthyroidism, adenoma)
- Before and after radioiodine therapy
- Newborns and infants (especially premature infants) due to risk of hypothyroidism
- Dermatitis herpetiformis
Drug Interactions
Lithium
Concurrent use may enhance the hypothyroid effects of lithium, especially with extensive application.
Octenidine
Concomitant use may cause transient dark discoloration.
Hydrogen peroxide
Incompatible, as hydrogen peroxide can inactivate povidone-iodine, reducing its efficacy.
Silver sulfadiazine
Concomitant use may reduce the antiseptic efficacy of both agents.
Storage
Store below 30°C. Protect from light, heat, and moisture. Keep out of reach of children. Do not freeze.
Overdose
Accidental ingestion of large quantities or extensive topical application on broken skin may lead to systemic iodine toxicity, presenting as metabolic acidosis, renal failure, and thyroid dysfunction. Treatment is symptomatic and supportive, including gastric lavage if ingested and monitoring of electrolyte and thyroid levels.
Pregnancy & Lactation
Use with caution and only if clearly needed. Extensive or prolonged use during pregnancy and lactation may lead to significant systemic iodine absorption, potentially affecting fetal or infant thyroid function. Monitor thyroid function if used.

YesClinical Trials
Povidone-iodine has been extensively studied and used clinically for decades. Its efficacy as a broad-spectrum antiseptic is well-established through numerous clinical trials and widespread medical use.
Lab Monitoring
- Thyroid function tests (T3, T4, TSH) in cases of extensive or prolonged use, especially in vulnerable populations (e.g., infants, patients with renal impairment or thyroid disease).
Doctor Notes
- Emphasize proper wound hygiene and application technique to patients.
- Be mindful of potential systemic iodine absorption in vulnerable populations (neonates, renal failure, thyroid disorders) and during extensive/prolonged use.
- Counsel patients on signs of allergic reactions or thyroid dysfunction to report promptly.
Patient Guidelines
- For external use only. Do not ingest.
- Avoid contact with eyes, ears, and mucous membranes. If contact occurs, rinse thoroughly with water.
- Do not use on large areas of broken or severely burnt skin without medical advice.
- Discontinue use if signs of irritation, redness, or swelling occur and consult a doctor.
- Do not apply to deep wounds or puncture wounds without consulting a doctor.
Missed Dose Advice
If a dose is missed, apply it as soon as remembered. Do not apply a double dose to make up for the forgotten application.
Driving Precautions
Povidone-iodine ointment is for topical use and is not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
- Maintain good hygiene practices around wounds.
