Description
Overview of the medicine
Povidone-iodine 10% w/v solution is an antiseptic agent used to prevent and treat skin infections in minor cuts, scrapes, and burns. It is also used for pre-operative skin preparation and for general skin disinfection.
Uses & Indications
Dosage
Adults
Apply undiluted solution to the affected skin area as needed. Allow to dry. May be repeated up to several times daily.
Elderly
No specific dosage adjustment required; use as per adult dosage.
Renal_impairment
Use with caution in patients with renal impairment, especially if extensive or prolonged use is anticipated, due to potential for systemic iodine absorption.
How to Take
For external use only. Apply topically to the skin. Do not swallow. Avoid contact with eyes.
Mechanism of Action
Povidone-iodine exerts its antimicrobial action through the sustained release of free iodine. The free iodine rapidly oxidizes microbial components, leading to the death of bacteria, fungi, viruses, and protozoa. It has a broad spectrum of activity.
Pharmacokinetics
Onset
Rapid, within seconds to minutes of application.
Excretion
Mainly through the kidneys in urine, with some fecal excretion.
Half life
Systemic iodine has a half-life of approximately 1-2 days, but for topical application, systemic half-life is less relevant due to minimal absorption.
Absorption
Minimal systemic absorption occurs through intact skin. Significant absorption can occur from damaged skin (e.g., burns), mucous membranes, or large wound surfaces, potentially leading to increased serum iodide levels.
Metabolism
Absorbed iodide is metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to iodine or povidone
- Patients with thyroid disorders (e.g., hyperthyroidism, goiter), especially if used extensively
- Pre-term neonates and infants under 6 months due to risk of iodine absorption
- Extensive burns (risk of systemic absorption)
Drug Interactions
Octenidine
Use with octenidine-containing antiseptics may cause transient dark discoloration.
Mercury-containing compounds
Concurrent use may form caustic mercury iodide compounds.
Hydrogen peroxide, silver sulfadiazine, taurolidine
These compounds can inactivate povidone-iodine and vice versa.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause serious systemic effects unless applied over large areas of broken skin or ingested. Ingestion or extensive absorption can lead to systemic iodine toxicity, symptoms include gastrointestinal distress, metabolic acidosis, thyroid dysfunction, and renal impairment. Management is symptomatic and supportive.
Pregnancy & Lactation
Use with caution. Although systemic absorption is minimal, extensive use during pregnancy or lactation may lead to iodine overload in the fetus/neonate and potentially affect thyroid function. Consult a doctor.
