Description
Overview of the medicine
Povimed 10% w/v Solution is a broad-spectrum antiseptic primarily used for disinfection of skin, wounds, and for pre-operative skin preparation. It contains povidone-iodine, which effectively kills bacteria, viruses, fungi, and protozoa.
Uses & Indications
Dosage
Adults
Apply undiluted solution directly to the affected skin area as needed. Allow to dry. Can be used several times a day.
Elderly
No special dosage adjustment required for elderly patients.
Renal_impairment
Use with caution in patients with severe renal impairment, especially if extensive or prolonged use, due to potential systemic iodine absorption.
How to Take
For external use only. Clean the affected area, then apply a small amount of Povimed solution. Allow to air dry. Do not bandage tightly unless advised by a healthcare professional.
Mechanism of Action
Povidone-iodine releases free iodine, which acts as an oxidizing agent. It rapidly penetrates microbial cells and disrupts proteins, nucleic acids, and other cellular components, leading to cell death. It has a broad spectrum of antimicrobial activity.
Pharmacokinetics
Onset
Rapid onset, typically within seconds to minutes of application.
Excretion
Mainly renal excretion for any absorbed iodine.
Half life
Not directly relevant for topical use, as systemic levels are usually negligible. Absorbed iodine is excreted rapidly.
Absorption
Minimal systemic absorption through intact skin. Absorption may increase with extensive skin damage or prolonged use over large areas.
Metabolism
Absorbed iodine is metabolized in the body and excreted. Not significant for typical topical application.
Side Effects
Contraindications
- Known hypersensitivity to iodine or povidone.
- Thyroid disorders (e.g., hyperthyroidism, goiter) due to potential iodine absorption.
- Before and after radio-iodine therapy.
- In neonates and very young infants, especially on large areas of broken skin, due to risk of systemic iodine absorption and thyroid effects.
Drug Interactions
Lithium
Avoid extensive use in patients receiving lithium therapy, as combined effects may lead to hypothyroidism.
Other antiseptics
Do not use with mercury-containing antiseptics (e.g., mercurochrome) as it may form corrosive compounds. Avoid concomitant use with other topical preparations like hydrogen peroxide, silver sulfadiazine, or octenidine as they may reduce the efficacy of povidone-iodine.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Accidental ingestion or extensive topical application over large areas for prolonged periods can lead to systemic iodine toxicity, presenting as thyroid dysfunction (hypothyroidism or hyperthyroidism), metabolic acidosis, and renal impairment. Treatment is symptomatic and supportive, including gastric lavage for ingestion and monitoring thyroid function.
Pregnancy & Lactation
Use with caution and only if clearly needed. Extensive use, especially on large areas or broken skin, may lead to significant iodine absorption, potentially affecting fetal or infant thyroid function. Consult a doctor.
