Description
Overview of the medicine
Prazopress 2 mg Tablet contains Prazosin, an alpha-1 blocker used to treat high blood pressure (hypertension) by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Initial dose: 0.5 mg two or three times daily. Gradually increase the dose based on patient response to a usual maintenance dose of 3-20 mg daily in divided doses. Maximum recommended dose is typically 20 mg/day, but some sources mention up to 40 mg/day for severe cases. The 2 mg tablet is often used in the titration or maintenance phase.
Elderly
Similar to adult dosage, but initiate with lower doses (e.g., 0.5 mg once or twice daily) and titrate slowly due to potential for increased sensitivity to hypotensive effects.
Renal_impairment
No specific dose adjustment is generally required, but close monitoring is advised. Initiate with lower doses and titrate carefully.
How to Take
Take orally with water, preferably at the same time each day. The first dose, especially, should be taken at bedtime to minimize the risk of 'first-dose' orthostatic hypotension.
Mechanism of Action
Prazosin selectively blocks post-synaptic alpha-1 adrenergic receptors, resulting in vasodilation (relaxation of blood vessels). This reduces peripheral vascular resistance and venous return, leading to a decrease in blood pressure.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Primarily excreted in bile and feces (approximately 90%), with a small amount excreted in urine (less than 10%).
Half life
Approximately 2-4 hours.
Absorption
Well absorbed from the gastrointestinal tract, with peak plasma concentrations occurring 2-3 hours after oral administration.
Metabolism
Extensively metabolized in the liver via demethylation and conjugation, mainly to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Prazosin or other quinazolines
- History of orthostatic hypotension with previous alpha-blocker therapy
Drug Interactions
NSAIDs
May reduce the antihypertensive effect of Prazosin.
Other Antihypertensives
Additive hypotensive effects may occur when Prazosin is co-administered with other antihypertensive agents. Dose reduction of Prazosin or the other agent may be necessary.
PDE5 Inhibitors (e.g., Sildenafil)
Concurrent use may result in symptomatic hypotension due to additive vasodilatory effects.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include marked hypotension and drowsiness. Treatment involves supportive measures, fluid replacement to raise blood pressure, and vasopressors if necessary. Dialysis is not effective.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Prazosin is excreted in small amounts in breast milk; use with caution in nursing mothers.

Clinical Trials
Extensive clinical trials have established the efficacy and safety of Prazosin in the treatment of hypertension and BPH symptoms.
Lab Monitoring
- Regular monitoring of blood pressure, especially during initiation and dose titration.
Doctor Notes
- Counsel patients on the 'first-dose phenomenon' to minimize syncopal episodes.
- Monitor blood pressure regularly, especially standing blood pressure.
- Consider Prazosin for patients with co-existing BPH or Raynaud's phenomenon where an alpha-blocker is suitable.
Patient Guidelines
- Take exactly as prescribed by your doctor.
- Be aware of 'first-dose' effect (dizziness, lightheadedness) which is more common after the first dose or when the dose is increased. Take the first dose at bedtime.
- Avoid sudden changes in posture, especially standing up quickly from a sitting or lying position, to prevent dizziness.
- Do not stop taking this medicine abruptly without consulting your doctor, as it may cause your blood pressure to rise.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Prazosin may cause dizziness or drowsiness, especially when starting treatment or after a dose increase. Avoid driving or operating heavy machinery until you know how the medicine affects you.
Lifestyle Advice
- Maintain a healthy diet low in sodium and saturated fats.
- Engage in regular physical activity as advised by your doctor.
- Limit or avoid alcohol consumption.
- Manage stress through relaxation techniques.
