Overview of the medicine
Pregabalin is a gamma-aminobutyric acid (GABA) analogue used to treat neuropathic pain, fibromyalgia, partial-onset seizures, and generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Typically initiated at 75 mg twice daily or 50 mg three times daily, titrated up to 150 mg twice daily (300 mg/day) or 100 mg three times daily, depending on indication and patient response. Max 600 mg/day.
Elderly
Dose reduction often needed due to age-related decrease in renal function. Starting dose may be lower.
Renal_impairment
Dosage adjustment is required based on creatinine clearance. For CrCl 30-60 mL/min, maximum dose 300 mg/day; for CrCl 15-30 mL/min, maximum dose 150 mg/day; for CrCl <15 mL/min, maximum dose 75 mg/day.
How to Take
Pregaben capsules are for oral administration. They can be taken with or without food.
Mechanism of Action
Pregabalin binds to the alpha2-delta (α2δ) subunit of voltage-gated calcium channels in the CNS, reducing the release of several neurotransmitters, including glutamate, norepinephrine, substance P, and calcitonin gene-related peptide. This reduces neuronal excitability.

Onset
Pain relief within 1 week for some conditions, anxiety relief within 1 week.
Excretion
Primarily excreted unchanged in urine (approximately 98% of the dose).
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration, bioavailability >90%. Peak plasma concentrations achieved within 1.5 hours.
Metabolism
Negligible hepatic metabolism (less than 0.1% of the dose is metabolized).

- Hypersensitivity to pregabalin or any component of the formulation.
Drug Interactions
ACE inhibitors
Co-administration may lead to an increased risk of angioedema.
Thiazolidinediones
Co-administration may result in an increased risk of weight gain and peripheral edema.
CNS depressants (opioids, benzodiazepines, ethanol)
May potentiate the effects of alcohol and other CNS depressants, leading to increased sedation and respiratory depression.

Overdose
Symptoms of overdose include somnolence, confusion, agitation, and restlessness. Management is supportive care, including gastric lavage if ingestion is recent. Hemodialysis may be beneficial in severe cases.
