Description
Overview of the medicine
Pregabalin is an anticonvulsant and neuropathic pain agent used to treat neuropathic pain associated with diabetic peripheral neuropathy, postherpetic neuralgia, and spinal cord injury. It is also used as adjunctive therapy for partial onset seizures in adults and for the management of fibromyalgia and generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Initial dose 75 mg twice daily or 50 mg three times daily (150 mg/day). May be increased to 150 mg twice daily or 100 mg three times daily (300 mg/day) within 1 week based on response and tolerability. Max dose usually 300 mg twice daily (600 mg/day).
Elderly
No specific dose reduction required based on age alone, but renal function should be assessed, and dose adjusted accordingly.
Renal_impairment
Dose adjustment required based on creatinine clearance. For CrCl 30-60 mL/min: 75 mg once daily or 25-50 mg twice daily. For CrCl <15 mL/min: 25 mg once daily.
How to Take
Take orally with or without food. Swallow the capsule whole with water.
Mechanism of Action
Pregabalin binds with high affinity to the alpha2-delta (α2δ) subunit of voltage-gated calcium channels in the central nervous system, which leads to a reduction in the release of several excitatory neurotransmitters.
Pharmacokinetics
Onset
Onset of analgesic effect can be seen within 1 week for some conditions, full effect may take longer. Anxiolytic effects may appear within 1 week.
Excretion
Primarily excreted unchanged in the urine; renal excretion is the major elimination pathway.
Half life
Approximately 6.3 hours.
Absorption
Rapidly and well absorbed after oral administration, with peak plasma concentrations occurring within 1 hour. Oral bioavailability is ≥90% and is independent of dose.
Metabolism
Negligible systemic metabolism; less than 0.1% of the dose is excreted as metabolites.
Side Effects
Contraindications
- Hypersensitivity to pregabalin or any of the excipients.
Drug Interactions
Thiazolidinedione antidiabetic agents
May increase the risk of weight gain and peripheral edema when co-administered.
CNS depressants (e.g., opioids, benzodiazepines, alcohol)
May potentiate the effects of alcohol and other CNS depressants, leading to increased sedation, dizziness, and cognitive impairment. Concomitant use should be approached with caution.
Storage
Store in a cool and dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and, rarely, coma. Treatment is supportive and symptomatic; hemodialysis may be beneficial in cases of severe overdose.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pregabalin is excreted in breast milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
