Description
Overview of the medicine
Pregmin-Z 50 mg Capsule is a nutritional supplement combining elemental iron, folic acid, and elemental zinc, primarily used to treat and prevent various types of anemia, especially during pregnancy and lactation, and to support overall health.
Uses & Indications
Dosage
Adults
1 capsule once daily, or as directed by physician.
Elderly
Same as adults, or as directed by physician, with caution for underlying conditions.
Renal_impairment
Use with caution. Dosage adjustment may be required in severe cases, consult physician.
How to Take
Take orally with water, preferably after food to minimize gastrointestinal upset.
Mechanism of Action
Iron is crucial for hemoglobin formation and oxygen transport. Folic acid is essential for DNA synthesis and red blood cell maturation. Zinc is vital for immune function, wound healing, and growth. Together, they address deficiencies leading to anemia and support various metabolic processes.
Pharmacokinetics
Onset
Improvement in symptoms typically within weeks, full hematological response in 1-2 months.
Excretion
Iron mainly through feces, small amounts in urine/sweat. Folic acid primarily through urine. Zinc primarily through feces, minimal via urine.
Half life
Iron is stored, not excreted rapidly. Folic acid has a plasma half-life of several hours. Zinc also has a variable half-life depending on body stores.
Absorption
Iron absorption is influenced by diet and iron status, typically 10-30%. Folic acid is readily absorbed from the GI tract. Zinc absorption varies based on dietary factors and body needs.
Metabolism
Iron is primarily incorporated into hemoglobin or stored as ferritin/hemosiderin. Folic acid is converted to active forms (e.g., tetrahydrofolate). Zinc is largely protein-bound and metabolized into various enzymes.
Side Effects
Contraindications
- •Hypersensitivity to any component
- •Hemochromatosis or hemosiderosis
- •Non-iron deficiency anemia (e.g., pernicious anemia unless used with B12)
- •Repeated blood transfusions
Drug Interactions
Levothyroxine
Iron may interfere with absorption. Separate administration by several hours.
Bisphosphonates
Iron may interfere with absorption. Separate administration.
Antacids, Proton Pump Inhibitors
May reduce iron absorption due to altered gastric pH.
Tetracyclines, Quinolones (antibiotics)
Iron and zinc can reduce absorption of these antibiotics. Take iron supplement at least 2 hours before or 4-6 hours after.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms may include severe gastrointestinal upset (nausea, vomiting, diarrhea, abdominal pain), metabolic acidosis, shock, cardiovascular collapse, and liver damage (due to iron toxicity). Seek immediate medical attention. Treatment involves supportive care, gastric lavage, and chelation therapy if severe iron overdose.
Pregnancy & Lactation
Generally considered safe and beneficial during pregnancy and lactation for treating or preventing iron, folic acid, and zinc deficiencies. Always consult a physician.
