Description
Overview of the medicine
Ondansetron is an antiemetic drug primarily used to prevent nausea and vomiting caused by chemotherapy, radiation therapy, and surgery.
Uses & Indications
Dosage
Adults
For chemotherapy-induced nausea/vomiting: 8 mg 30 minutes before chemotherapy, then 8 mg every 8 hours for 1-2 days. For postoperative nausea/vomiting: 16 mg orally one hour before anesthesia.
Elderly
No dosage adjustment generally required, but caution in patients over 75 years due to increased half-life.
Renal_impairment
No dosage adjustment needed.
How to Take
Take orally with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Ondansetron is a selective 5-HT3 receptor antagonist. Serotonin 5-HT3 receptors are present both peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone. By blocking these receptors, ondansetron prevents the initiation of the vomiting reflex.

Onset
Oral: 30 minutes to 2 hours.
Excretion
Primarily eliminated via urine (about 5-10% as unchanged drug) and feces (metabolites).
Half life
Approximately 3-6 hours in adults.
Absorption
Well absorbed from the gastrointestinal tract, undergoes first-pass metabolism.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP1A2, CYP2D6, CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to ondansetron or any component of the formulation.
- Concomitant use with apomorphine.
Drug Interactions
Tramadol
May reduce analgesic effect and increase risk of serotonin syndrome.
Apomorphine
Concomitant use is contraindicated due to risk of profound hypotension and loss of consciousness.
Drugs that prolong QT interval
Increased risk of QT prolongation and Torsade de Pointes.
Phenytoin, Carbamazepine, Rifampicin
May decrease ondansetron plasma levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include visual disturbances, constipation, severe hypotension, and vagal episodes with transient AV block. Treatment is supportive and symptomatic; ECG monitoring is recommended.
Pregnancy & Lactation
Pregnancy: Use with caution; category B. Human data are inconclusive, but animal studies have not shown harm. Consult doctor. Lactation: It is not known whether ondansetron is excreted in human milk. Exercise caution when administered to a nursing woman.
