Description
Overview of the medicine
Omeprazole is a proton pump inhibitor used to reduce stomach acid production. It treats conditions like GERD, ulcers, and erosive esophagitis.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Duodenal Ulcer: 20 mg once daily for 2-4 weeks. Gastric Ulcer: 20-40 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial 60 mg once daily, adjusted based on response.
Elderly
No specific dose adjustment required, but caution advised due to potential comorbidities and reduced hepatic function.
Renal_impairment
No dose adjustment needed for most patients with renal impairment.
How to Take
Take orally, preferably in the morning before breakfast. Swallow whole with water. Do not chew, crush, or open the capsule.
Mechanism of Action
Selectively inhibits the H+/K+-ATPase enzyme system (proton pump) in gastric parietal cells, thereby blocking the final step of acid secretion.
Pharmacokinetics
Onset
Acid secretion inhibition begins within 1 hour, maximal effect seen after 2-4 days of continuous treatment.
Excretion
Mainly via urine (approx. 77%), some via feces (approx. 18%).
Half life
0.5-1 hour (short plasma half-life but long duration of action due to irreversible binding to the proton pump).
Absorption
Rapidly absorbed from the gastrointestinal tract, bioavailability approximately 30-40%. Peak plasma concentrations within 0.5-3.5 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation
- Concomitant use with nelfinavir (due to significant reduction in nelfinavir plasma levels)
Drug Interactions
Warfarin
Increased INR and bleeding risk due to altered metabolism.
Nelfinavir
Significant reduction in nelfinavir plasma levels, leading to loss of antiviral efficacy.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to CYP2C19 inhibition by omeprazole.
Methotrexate
Increased methotrexate levels, especially with high-dose methotrexate.
Diazepam, Phenytoin, Citalopram
Increased plasma levels of these drugs due to inhibition of CYP2C19.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach and sight of children.
Overdose
Symptoms of overdose are generally mild and include nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Pregnancy category C. Use only if clearly needed and the potential benefits outweigh the risks. Omeprazole is excreted in breast milk; use with caution in lactating mothers.

Lab Monitoring
- Serum magnesium levels (especially with prolonged use or co-administration with diuretics)
- Chromogranin A (CgA) levels (can interfere with neuroendocrine tumor diagnosis, discontinue PPIs at least 14 days before CgA measurement)
- Vitamin B12 levels (with prolonged use)
Doctor Notes
- Consider using the lowest effective dose for the shortest duration necessary to achieve therapeutic goals.
- Counsel patients on potential long-term risks associated with PPI use, particularly bone fractures and C. difficile infections.
- Periodically review the need for continued PPI therapy, as many patients may not require indefinite treatment.
Patient Guidelines
- Take exactly as prescribed by your doctor.
- Do not stop taking abruptly without consulting your doctor.
- Inform your doctor about all other medications, including over-the-counter drugs and herbal supplements, you are taking.
- Report any new or worsening symptoms to your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as remembered. If it's almost time for the next dose, skip the missed dose and resume the regular schedule. Do not double dose.
Driving Precautions
Omeprazole may cause dizziness or visual disturbances in some patients. If you experience these effects, avoid driving or operating machinery.
Lifestyle Advice
- Avoid spicy, fatty, and acidic foods that can trigger symptoms.
- Avoid alcohol and smoking, as they can irritate the stomach lining.
- Eat smaller, more frequent meals instead of large ones.
- Elevate the head of your bed to reduce nighttime reflux.
