Description
Overview of the medicine
Presnor 40 mg tablet contains Rosuvastatin, a medication used to lower high cholesterol and triglycerides in the blood. It helps reduce the risk of heart attack, stroke, and other heart-related complications.
Uses & Indications
Dosage
Adults
Initial dose typically 5-10 mg once daily. For Presnor 40 mg, it's usually for patients requiring higher lipid reduction or who have not achieved target goals on lower doses. Max dose 40 mg once daily.
Elderly
Initial dose 5 mg once daily for patients >70 years. Max 40 mg once daily.
Renal_impairment
For severe renal impairment (CrCl <30 mL/min), initial dose 5 mg once daily, max 10 mg once daily.
How to Take
Swallow the tablet whole with water. Can be taken with or without food, at any time of day, but should be taken at the same time each day.
Mechanism of Action
Rosuvastatin selectively inhibits HMG-CoA reductase, an enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis. This inhibition leads to a decrease in hepatic cholesterol synthesis and an increase in the number of LDL receptors on liver cells, enhancing the uptake and catabolism of LDL.
Pharmacokinetics
Onset
Lipid-lowering effects typically begin within 1 week, with 90% of maximum effect achieved by 2 weeks.
Excretion
Mainly excreted in feces (90%) as unchanged drug and metabolites; approximately 5% excreted in urine.
Half life
Approximately 19 hours.
Absorption
Approximately 20% absorbed from the gastrointestinal tract; absolute bioavailability is about 20%.
Metabolism
Limited metabolism, primarily by CYP2C9; approximately 10% is metabolized.
Side Effects
Contraindications
- Hypersensitivity to rosuvastatin or any component of the formulation
- Active liver disease, including unexplained persistent elevations of serum transaminases
- Pregnancy and lactation
- Severe renal impairment (CrCl <30 mL/min) at 40 mg dose
- Concomitant use with ciclosporin
Drug Interactions
Warfarin
May enhance anticoagulant effect; monitor INR.
Ciclosporin
Increases rosuvastatin exposure; avoid co-administration or use lower dose.
Gemfibrozil
Increases rosuvastatin exposure; avoid co-administration or use lower dose.
Erythromycin
Decreases rosuvastatin exposure; monitor lipid levels.
Oral Contraceptives
May increase levels of ethinyl estradiol and norgestrel; monitor for adverse effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. In case of overdose, provide supportive treatment and monitor liver function and CK levels. Hemodialysis is unlikely to be of benefit.

Numerous clinical trials (e.g., JUPITER, ASTEROID) have demonstrated the efficacy and safety of rosuvastatin in lipid lowering and cardiovascular risk reduction.
Lab Monitoring
- Liver function tests (ALT, AST) at baseline and periodically as clinically indicated.
- Creatine Kinase (CK) levels if muscle symptoms occur.
- Lipid profile (total cholesterol, LDL-C, HDL-C, triglycerides) at baseline and 4-12 weeks after initiation or dose titration, then periodically.
Doctor Notes
- Counsel patients on potential for muscle pain and weakness; emphasize reporting these symptoms promptly.
- Regular monitoring of lipid profile is crucial.
- Consider lower starting doses for elderly or renally impaired patients.
Patient Guidelines
- Take as directed by your doctor, usually once daily.
- Do not stop taking this medicine without consulting your doctor.
- Inform your doctor about any muscle pain, tenderness, or weakness, especially if accompanied by fever or dark urine.
- Avoid alcohol consumption during treatment.
