Description
Overview of the medicine
Prevastin 20 mg Capsule is an HMG-CoA reductase inhibitor (statin) used to lower high cholesterol and triglyceride levels in the blood. It helps reduce the risk of heart attack, stroke, and other cardiovascular problems by reducing the production of cholesterol in the liver.
Uses & Indications
Dosage
Adults
Initial dose is usually 10-20 mg once daily, preferably in the evening. Dose can be adjusted up to 40 mg daily based on lipid levels and patient response. Maximum dose is 80 mg daily.
Elderly
No specific dose adjustment is required in elderly patients, but caution is advised due to potential increased susceptibility to myopathy.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min), initial dose should be 10 mg once daily and should not exceed 20 mg daily.
How to Take
Prevastin capsule should be taken orally once daily, with or without food, preferably in the evening. Swallow the capsule whole with a glass of water; do not crush, chew, or open it.
Mechanism of Action
Prevastin competitively inhibits HMG-CoA reductase, an enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis. This inhibition reduces cholesterol production in the liver, leading to an increase in LDL receptors on liver cells, which enhances the uptake and breakdown of LDL-cholesterol from the bloodstream.
Pharmacokinetics
Onset
Lipid-lowering effects begin within 2 weeks, with maximal effects seen in 4-6 weeks.
Excretion
Excreted primarily via feces (approx. 70%) and urine (approx. 30%).
Half life
Approximately 1.5-2 hours, but duration of HMG-CoA reductase inhibition is longer due to active metabolites.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is moderate, typically around 30-40%.
Metabolism
Primarily metabolized in the liver, largely by sulfation and other pathways, not extensively by CYP450 enzymes.
Side Effects
Contraindications
- Active liver disease or unexplained persistent elevations of serum transaminases.
- Pregnancy and lactation.
- Hypersensitivity to Prevastatin or any component of the formulation.
Drug Interactions
Colchicine
Increased risk of myopathy.
Cyclosporine
Increased plasma concentrations of Prevastatin, requiring dose reduction and careful monitoring.
Fibrates (e.g., Gemfibrozil)
Increased risk of myopathy and rhabdomyolysis. Concomitant use should be avoided or used with extreme caution.
Niacin (lipid-lowering doses)
Increased risk of muscle-related side effects, including myopathy and rhabdomyolysis.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for Prevastin overdose. In case of an overdose, treatment should be symptomatic and supportive. Close monitoring of liver function and CK levels is recommended.
Pregnancy & Lactation
Prevastin is contraindicated during pregnancy and breastfeeding. It can cause fetal harm. Women of childbearing potential should use adequate contraception during therapy. If pregnancy occurs, Prevastin should be discontinued immediately.

YesClinical Trials
Extensive clinical trials have demonstrated Prevastin's efficacy in lowering LDL-C and reducing major adverse cardiovascular events across various patient populations, including those with primary hypercholesterolemia and established cardiovascular disease.
Lab Monitoring
- Liver function tests (ALT, AST) before initiation and as clinically indicated or periodically (e.g., at 12 weeks after initiation or dose escalation, then annually).
- Creatine kinase (CK) levels if muscle pain, tenderness, or weakness occurs.
- Lipid panel (total cholesterol, LDL, HDL, triglycerides) at baseline and periodically to assess therapeutic response.
Doctor Notes
- Emphasize the importance of lifestyle modifications alongside pharmacotherapy for optimal lipid management.
- Counsel patients on muscle-related symptoms and the need for immediate reporting.
- Regular monitoring of liver function tests is crucial, especially during initiation and dose adjustments.
- Contraindicated in pregnancy and lactation; ensure effective contraception for women of childbearing potential.
Patient Guidelines
- Take Prevastin exactly as prescribed by your doctor, usually once daily in the evening.
- Do not stop taking Prevastin without consulting your doctor, even if you feel well.
- Report any unexplained muscle pain, tenderness, or weakness, especially if accompanied by fever or dark urine, to your doctor immediately.
- Inform your doctor about all other medications, supplements, and herbal products you are taking.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Prevastin is generally not expected to impair the ability to drive or operate machinery. However, if you experience dizziness or visual disturbances, you should avoid such activities until you are sure you can perform them safely.
Lifestyle Advice
- Maintain a low-cholesterol, low-fat diet as advised by your doctor or dietitian.
- Incorporate regular physical activity into your routine.
- Achieve and maintain a healthy weight.
- Avoid excessive alcohol consumption and stop smoking.
