Description
Overview of the medicine
Previp 50 mg Tablet is used to treat high blood pressure (hypertension) and to help protect the kidneys from damage due to diabetes. It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Initial dose: 50 mg once daily. May be increased to 100 mg once daily if needed. For patients with possible intravascular volume depletion, a lower initial dose of 25 mg once daily is recommended.
Elderly
No initial dosage adjustment is typically needed, but consider starting with 25 mg for very frail or volume-depleted patients.
Renal_impairment
No initial dosage adjustment is necessary for mild to moderate renal impairment. For severe renal impairment or dialysis, a lower starting dose (e.g., 25 mg) may be considered.
How to Take
Previp 50 mg Tablet should be taken orally with or without food, usually once daily. It is important to take it at about the same time each day.
Mechanism of Action
Losartan is an angiotensin II receptor blocker (ARB). It selectively blocks the binding of angiotensin II to the AT1 receptor found in many tissues (e.g., vascular smooth muscle, adrenal gland). This blockade prevents the vasoconstricting and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect begins within 6 hours, full effect within 3-6 weeks.
Excretion
Approximately 35% excreted in urine and 60% in feces, mainly as unchanged drug and its active metabolite.
Half life
Losartan: 2 hours; active metabolite (E-3174): 6-9 hours.
Absorption
Well absorbed after oral administration, with an oral bioavailability of about 33%. Peak plasma concentrations are reached in about 1 hour for Losartan and 3-4 hours for its active metabolite.
Metabolism
Undergoes extensive first-pass metabolism by CYP2C9 and CYP3A4 to an active carboxylic acid metabolite (E-3174), which is more potent than Losartan.
Side Effects
Contraindications
- Hypersensitivity to Losartan or any component of the formulation
- Pregnancy (especially second and third trimesters)
- Concomitant use with Aliskiren in patients with diabetes or renal impairment (GFR <60 ml/min/1.73m²)
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity have been reported. Monitor lithium levels carefully.
Diuretics (e.g., Furosemide)
May enhance the hypotensive effect, especially at initiation. Monitor blood pressure closely.
Other antihypertensive agents
Additive hypotensive effects may occur.
NSAIDs (e.g., Ibuprofen, Naproxen)
May attenuate the antihypertensive effect and increase the risk of renal function deterioration, especially in elderly, volume-depleted, or renally impaired patients.
Potassium-sparing diuretics (e.g., Spironolactone) or Potassium supplements
May increase serum potassium levels, leading to hyperkalemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are likely to include hypotension (low blood pressure) and tachycardia. Bradycardia could occur from parasympathetic stimulation. Treatment is symptomatic and supportive. If ingestion is recent, induce emesis or perform gastric lavage. Administer activated charcoal to reduce absorption. Monitor vital signs and fluid/electrolyte balance.
Pregnancy & Lactation
Pregnancy: Contraindicated during the second and third trimesters due to fetal toxicity. Discontinue Previp immediately if pregnancy is detected. Lactation: Not recommended. It is unknown if Losartan is excreted in human milk. A decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
