Overview of the medicine
Pristil 5 mg Tablet contains an active ingredient (hypothetically Amlodipine) used to treat high blood pressure (hypertension) and certain types of chest pain (angina). It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Initial dose is 5 mg once daily. Max 10 mg once daily. For elderly or hepatic impairment, initial dose 2.5 mg once daily.
Elderly
Initial dose 2.5 mg once daily. Dose adjustments should be made with caution.
Renal_impairment
No dosage adjustment required for renal impairment.
How to Take
Take Pristil tablet orally with or without food, usually once daily. Swallow the tablet whole with water.
Mechanism of Action
Pristil (hypothetically Amlodipine) is a dihydropyridine calcium antagonist (calcium ion antagonist or slow-channel blocker) that inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. The contractile process of cardiac muscle and vascular smooth muscle is dependent upon the movement of extracellular calcium ions into these cells through specific ion channels. Pristil inhibits calcium ion influx across the cell membrane selectively, with a greater effect on vascular smooth muscle cells than on cardiac muscle cells.

Onset
Antihypertensive effect begins within 24 hours.
Excretion
Primarily excreted in the urine (60%) as inactive metabolites, with 20-25% excreted in the feces.
Half life
Approximately 30-50 hours.
Absorption
Well absorbed from the gastrointestinal tract, with peak plasma concentrations occurring 6-12 hours after oral administration. Absolute bioavailability is estimated to be 64-80%.
Metabolism
Extensively metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Pristil or any dihydropyridine derivatives
- Severe hypotension
- Cardiogenic shock
- Obstruction of the outflow tract of the left ventricle (e.g., high grade aortic stenosis)
- Hemodynamically unstable heart failure after acute myocardial infarction
Drug Interactions
Simvastatin
Limit simvastatin dose to 20 mg daily when co-administered with Pristil.
Clarithromycin
Increased risk of hypotension and acute kidney injury.
CYP3A4 Inducers (e.g., Rifampicin)
May decrease Pristil exposure, requiring dose adjustment.
CYP3A4 Inhibitors (e.g., Ketoconazole, Erythromycin)
May increase Pristil exposure, requiring dose reduction.
Storage

Overdose
Overdose may lead to excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known if Pristil is excreted in human milk; caution should be exercised when administered to a nursing woman.
