Description
Overview of the medicine
Pristin OL is an antipsychotic medication containing Olanzapine, primarily used to treat schizophrenia and bipolar disorder (manic or mixed episodes), and for the maintenance treatment of bipolar I disorder.
Uses & Indications
Dosage
Adults
For schizophrenia, recommended starting dose is 10 mg once daily. For bipolar mania, initial dose is 10-15 mg once daily. Dose adjustments typically occur in 5 mg increments at intervals of not less than 24 hours.
Elderly
A lower starting dose (e.g., 5 mg/day) should be considered for elderly patients, with careful titration based on clinical response and tolerability.
Renal_impairment
No specific dose adjustment is routinely required in patients with mild to moderate renal impairment, but caution is advised. Severe impairment requires careful monitoring.
How to Take
Pristin OL tablets should be taken orally once daily, with or without food. It is recommended to take the tablet at the same time each day.
Mechanism of Action
Olanzapine acts as an antagonist at multiple receptors including D2 dopamine receptors and 5-HT2A serotonin receptors. Its antipsychotic action is believed to be mediated through a combination of dopamine and serotonin antagonism.
Pharmacokinetics
Onset
Initial symptom improvement can be seen within hours, but full therapeutic effects may take several weeks.
Excretion
Approximately 57% of the dose is excreted in the urine and 30% in the feces, mainly as metabolites.
Half life
The elimination half-life ranges from 21 to 54 hours (mean 33 hours).
Absorption
Well absorbed after oral administration, reaching peak plasma concentrations in 5-8 hours. Bioavailability is not affected by food.
Metabolism
Extensively metabolized in the liver, primarily through direct glucuronidation and CYP1A2-mediated oxidation. CYP2D6 is a minor pathway.
Side Effects
Contraindications
- Known hypersensitivity to Olanzapine or any component of the formulation.
- Patients with narrow-angle glaucoma.
Drug Interactions
CNS Depressants
Concurrent use with alcohol or other CNS depressants may cause additive sedative effects.
CYP1A2 Inducers
Carbamazepine and rifampicin can decrease olanzapine levels by inducing CYP1A2 activity, potentially reducing efficacy.
CYP1A2 Inhibitors
Fluvoxamine, a potent CYP1A2 inhibitor, can significantly increase olanzapine levels. Consider a lower olanzapine dose.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, tachycardia, extrapyramidal symptoms, slurred speech, and potentially coma. Management is supportive, including maintaining an open airway, oxygenation, and ventilation. Activated charcoal may be useful.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Olanzapine is excreted in human milk; therefore, breastfeeding is not recommended.
