Description
Overview of the medicine
Probal-DX 600 mg Tablet is a combination of Calcium Carbonate and Vitamin D3 (Cholecalciferol) used to prevent and treat calcium and vitamin D deficiencies, primarily for maintaining bone health and preventing osteoporosis.
Uses & Indications
Dosage
Adults
1 to 2 tablets daily, or as directed by a physician.
Elderly
Same as adult dosage, but caution should be exercised in patients with renal impairment.
Renal_impairment
Use with caution; dose adjustment may be necessary based on calcium and phosphate levels. Avoid in severe renal impairment.
How to Take
Oral administration. Can be taken with or without food, but taking with meals may improve absorption and reduce gastric upset.
Mechanism of Action
Calcium Carbonate provides elemental calcium, an essential mineral for bone formation and maintenance, nerve transmission, muscle contraction, and many other bodily functions. Vitamin D3 (Cholecalciferol) enhances the absorption of calcium from the gastrointestinal tract and regulates calcium and phosphate homeostasis.
Pharmacokinetics
Onset
Effects on bone density are gradual, over weeks to months.
Excretion
Calcium is excreted mainly via feces, with some renal excretion. Vitamin D metabolites are primarily excreted in the bile and feces.
Half life
Calcium's half-life varies. Cholecalciferol has a half-life of about 15-20 days.
Absorption
Calcium carbonate is absorbed primarily in the small intestine, requiring gastric acid for dissolution. Cholecalciferol is readily absorbed from the small intestine, especially in the presence of bile salts.
Metabolism
Calcium is not metabolized. Cholecalciferol is hydroxylated in the liver to 25-hydroxyvitamin D3 and then in the kidneys to the active form, 1,25-dihydroxyvitamin D3.

Contraindications
- Hypercalcemia (high blood calcium levels)
- Hypercalciuria (high urine calcium levels)
- Severe renal impairment
- Known hypersensitivity to calcium carbonate, cholecalciferol, or any components of the formulation
Drug Interactions
Levothyroxine
Calcium can impair absorption; administer at least 4 hours apart.
Bisphosphonates
Calcium can reduce their absorption; administer at least 30 minutes apart.
Cardiac glycosides
Increased risk of arrhythmias.
Thiazide diuretics
May increase the risk of hypercalcemia.
Tetracyclines & Quinolones
Calcium can decrease their absorption; administer at least 2 hours before or 4-6 hours after.
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of reach of children.

Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, polyuria, polydipsia, muscle weakness, kidney stones). Treatment involves discontinuing the product, hydration, and medical management of hypercalcemia.
