Description
Overview of the medicine
Probet 0.05% Cream contains Clobetasol Propionate, a very potent corticosteroid. It is used for the short-term treatment of inflammatory and pruritic manifestations of moderate to severe corticosteroid-responsive dermatoses, such as severe psoriasis, eczema, lichen planus, and discoid lupus erythematosus.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin areas once or twice daily for a maximum of 2-4 weeks. Total weekly dosage should not exceed 50g.
Elderly
Use with caution. Dosage similar to adults, but monitor for systemic side effects.
Renal_impairment
No specific dosage adjustment required, but monitor for systemic effects if absorption is significant.
How to Take
For external use only. Apply a thin layer to the affected skin area and rub in gently. Do not cover with occlusive dressings unless directed by a physician. Wash hands thoroughly after application.
Mechanism of Action
Clobetasol propionate is a highly potent corticosteroid that works by activating natural substances in the skin to reduce swelling, redness, and itching. It binds to glucocorticoid receptors, inhibiting the release of inflammatory mediators like prostaglandins and leukotrienes, thereby exerting anti-inflammatory, antipruritic, and vasoconstrictive effects.
Pharmacokinetics
Onset
Rapid onset of anti-inflammatory and antipruritic effects.
Excretion
Excreted primarily by the kidneys, with some excretion in the bile.
Half life
Once absorbed systemically, Clobetasol Propionate is metabolized primarily in the liver. Its systemic half-life is not precisely defined for topical administration, but systemic effects are related to the absorbed amount.
Absorption
Systemic absorption can occur, especially with prolonged use, application to large areas, broken skin, or with occlusive dressings. The extent of percutaneous absorption is determined by many factors including the vehicle, the integrity of the epidermal barrier, and the use of occlusive dressings.
Metabolism
Metabolized primarily in the liver, similar to other corticosteroids, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any components of the formulation
- Bacterial, fungal, or viral infections of the skin (e.g., herpes simplex, chickenpox)
- Rosacea, perioral dermatitis, acne vulgaris
- Pruritus without inflammation
- Children under 1 year of age
Drug Interactions
Other topical steroids
Avoid concomitant use with other corticosteroids to prevent cumulative systemic effects.
CYP3A4 inhibitors (e.g., Ritonavir, Itraconazole)
May increase systemic exposure to Clobetasol Propionate, leading to increased risk of systemic side effects. Use with caution.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Acute overdose is unlikely. In the event of chronic overdose or misuse, symptoms of hypercorticism (e.g., Cushing's syndrome) may occur due to systemic absorption. Treatment involves gradual withdrawal of the drug, under medical supervision, and symptomatic management.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether topical administration of corticosteroids could result in sufficient systemic absorption to produce detectable quantities in breast milk. Use with caution in nursing mothers.

Clinical Trials
Clobetasol Propionate has undergone extensive clinical trials demonstrating its efficacy and safety for various corticosteroid-responsive dermatoses. Studies confirm its potent anti-inflammatory and antipruritic effects, with outcomes varying based on duration of use, surface area, and patient population.
Lab Monitoring
- Not routinely required for short-term topical use.
- If prolonged use or extensive application: monitor plasma cortisol levels and urinary free cortisol for evidence of HPA axis suppression.
Doctor Notes
- Emphasize short-term use (max 2-4 weeks) and lowest effective dose.
- Advise against use on face, groin, or underarms without specific instruction.
- Educate patients on signs of systemic absorption (e.g., rapid weight gain, moon face) and local side effects (e.g., skin thinning).
- Consider alternative, less potent steroids for maintenance therapy or less severe conditions.
Patient Guidelines
- Use only as directed by your doctor. Do not use for longer than prescribed.
- Apply a very thin layer to the affected area. Do not use on the face, groin, or underarms unless specifically instructed by your doctor.
- Avoid contact with eyes. If cream gets into eyes, rinse thoroughly with water.
- Do not cover the treated area with bandages or other dressings unless advised by your doctor.
- Wash your hands before and after applying the cream.
Missed Dose Advice
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not apply a double dose to make up for a missed one.
Driving Precautions
Probet 0.05% Cream is for topical use and is not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
- Avoid scratching the affected area to prevent further irritation and infection.
- Keep the skin moisturized with non-medicated emollients after steroid treatment, if recommended by your doctor.
