Description
Overview of the medicine
Progan 5 mg Oral Solution contains Promethazine Hydrochloride, a first-generation antihistamine with sedative, antiemetic, and anticholinergic properties. It is used to treat allergic reactions, motion sickness, and as a short-term sedative.
Uses & Indications
Dosage
Adults
Allergy: 10-25 mg at bedtime or 6.25-12.5 mg three times daily. Motion sickness: 25 mg 30-60 minutes before travel, repeat 8-12 hours later if needed. Nausea/Vomiting: 25 mg orally, then 12.5-25 mg every 4-6 hours as needed.
Elderly
Lower doses may be required due to increased sensitivity to anticholinergic effects and CNS depression.
Renal_impairment
No specific dosage adjustments are typically provided, but caution and reduced dosage may be considered due to potential accumulation of metabolites.
How to Take
Take orally, with or without food. Measure the dose accurately using the provided measuring device (e.g., spoon or syringe).
Mechanism of Action
Promethazine acts as a potent H1-receptor antagonist, blocking the effects of histamine. It also has significant anticholinergic activity, contributing to its antiemetic and sedative effects, and antagonizes dopamine receptors.
Pharmacokinetics
Onset
Oral: 20 minutes
Excretion
Mainly excreted in urine as inactive metabolites, with some fecal excretion.
Half life
10-14 hours (variable, range 7-12 hours)
Absorption
Readily absorbed from the gastrointestinal tract.
Metabolism
Extensively metabolized in the liver, primarily by hydroxylation and N-demethylation.
Side Effects
Contraindications
- Hypersensitivity to promethazine or other phenothiazines
- Patients in a comatose state or with severe CNS depression
- Children younger than 2 years of age (due to risk of fatal respiratory depression)
- Lower respiratory tract symptoms, including asthma
Drug Interactions
Epinephrine
Promethazine may reverse the vasopressor effect of epinephrine.
Monoamine Oxidase Inhibitors (MAOIs)
Prolonged and intensified anticholinergic and CNS depressant effects.
Anticholinergic drugs (e.g., tricyclic antidepressants, atropine)
Increased anticholinergic side effects (e.g., dry mouth, blurred vision, urinary retention).
CNS Depressants (e.g., alcohol, sedatives, hypnotics, tranquilizers)
Increased CNS depression, drowsiness, and respiratory depression.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of overdose include severe drowsiness, dizziness, fainting, dry mouth, blurred vision, flushing, hallucinations, seizures, and respiratory depression. Treatment is supportive, including maintaining an open airway, providing respiratory support, and managing symptoms.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Not recommended during the last trimester due to potential for withdrawal symptoms in newborns. Excreted in breast milk; caution should be exercised when administered to a nursing mother.
