Description
Overview of the medicine
Progavi 500 mg Suspension is an antiviral medication containing Ganciclovir, used for the treatment and prevention of cytomegalovirus (CMV) infections, particularly in immunocompromised patients.
Uses & Indications
Dosage
Adults
CMV Retinitis: Induction 1000 mg three times daily for 21 days with food. Maintenance 1000 mg three times daily with food.
Elderly
Dosage adjustment required based on renal function.
Renal_impairment
Significant dose reduction required; consult dosing guidelines based on creatinine clearance.
How to Take
Administer orally with food to enhance absorption. Shake the suspension well before each use. Measure dose accurately using an oral syringe.
Mechanism of Action
Ganciclovir is a synthetic guanine derivative that inhibits viral DNA synthesis. It is phosphorylated to Ganciclovir triphosphate, which then competitively inhibits viral DNA polymerase and is incorporated into viral DNA, leading to chain termination.
Pharmacokinetics
Onset
Within 24-48 hours for clinical effect, peak plasma concentration usually 1-3 hours post-dose.
Excretion
Primarily renally excreted (90% as unchanged drug).
Half life
Approximately 2.9 hours (for Ganciclovir), longer in renal impairment.
Absorption
Oral bioavailability is low (5-9%) but increases with food.
Metabolism
Minimal hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to Ganciclovir or Valganciclovir.
- Neutropenia (absolute neutrophil count <500 cells/µL) or thrombocytopenia (platelet count <25,000 cells/µL).
Drug Interactions
Didanosine
Increased Didanosine plasma concentrations.
Zidovudine
Increased hematologic toxicity.
Imipenem/Cilastatin
Increased risk of seizures.
Mycophenolate Mofetil
Increased concentrations of both drugs, increased risk of toxicity.
Storage
Store at room temperature (20°C to 25°C), away from moisture and heat. Do not freeze. Keep out of reach of children.
Overdose
Overdose can lead to increased myelosuppression, renal toxicity, and gastrointestinal disturbances. Management is supportive and includes hydration and hemodialysis.
Pregnancy & Lactation
Pregnancy Category D: May cause fetal harm. Avoid during pregnancy unless potential benefits outweigh risks. Not recommended during breastfeeding; consider stopping breastfeeding or the drug.

Lab Monitoring
- Complete blood count (CBC) with differential, at least weekly during induction therapy and regularly during maintenance.
- Serum creatinine and BUN to monitor renal function.
- Liver function tests (LFTs) periodically.
Doctor Notes
- Monitor CBC and renal function closely, especially during induction therapy.
- Counsel patients on adherence due to critical nature of CMV infections.
- Consider dose adjustments for concomitant nephrotoxic drugs.
Patient Guidelines
- Take Progavi with food to maximize absorption.
- Complete the full course of treatment as prescribed, even if symptoms improve.
- Report any signs of infection (fever, chills) or unusual bruising/bleeding to your doctor immediately.
- Maintain adequate hydration.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
May cause dizziness, seizures, or confusion. Patients should be cautioned about operating machinery or driving until they are reasonably certain that Progavi does not affect them adversely.
Lifestyle Advice
- Avoid activities that may increase the risk of injury or bleeding due to potential thrombocytopenia.
- Practice good hygiene to prevent infections.
