Description
Overview of the medicine
Prolan 60 mg capsule is an antidepressant medication belonging to the Selective Serotonin Reuptake Inhibitor (SSRI) class. It is primarily used to treat major depressive disorder, obsessive-compulsive disorder, panic disorder, and generalized anxiety disorder by balancing serotonin levels in the brain.
Uses & Indications
Dosage
Adults
Initial dose 20 mg once daily, may be increased to 60 mg daily for certain conditions or if needed, usually taken in the morning.
Elderly
Lower initial doses (e.g., 10-20 mg daily) are recommended, with careful titration due to increased sensitivity.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Take orally, with or without food, usually once daily in the morning. Swallow the capsule whole with water.
Mechanism of Action
Prolan selectively inhibits the reuptake of serotonin (5-HT) by the presynaptic neuron, thereby increasing the concentration of serotonin in the synaptic cleft. This leads to enhanced serotonergic neurotransmission in the central nervous system.
Pharmacokinetics
Onset
Initial effects within 1-2 weeks; full antidepressant effect in 4-6 weeks.
Excretion
Excreted mainly through urine as metabolites.
Half life
Approximately 1-4 days (for parent drug), active metabolites have longer half-lives (e.g., 7-10 days).
Absorption
Well absorbed orally, peak plasma concentrations reached in 4-8 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 and other isoenzymes.
Side Effects
Contraindications
- Hypersensitivity to Prolan or any of its excipients
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI
- Concomitant use with thioridazine or pimozide
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes).
Warfarin
Potentiates anticoagulant effect, leading to increased bleeding risk.
Linezolid
Risk of serotonin syndrome.
Benzodiazepines
Increased plasma concentrations of some benzodiazepines.
Tricyclic Antidepressants (TCAs)
Increased plasma levels of TCAs, potentially leading to increased side effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, dizziness, agitation, tremor, and rarely, seizures or serotonin syndrome. Treatment is supportive and symptomatic; gastric lavage or activated charcoal may be considered.
Pregnancy & Lactation
Use in pregnancy only if potential benefit justifies potential risk to the fetus. SSRIs are excreted in breast milk; consult a doctor before use during lactation.
