Description
Overview of the medicine
Prolok 40 mg Injection contains Pantoprazole, a proton pump inhibitor used to reduce stomach acid production, primarily for conditions like peptic ulcers, GERD, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
40 mg once daily, intravenously. For Zollinger-Ellison Syndrome, initial dose may be higher and adjusted based on acid output.
Elderly
No specific dose adjustment is required for elderly patients.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Administered intravenously over 2-15 minutes, or as a continuous infusion. Reconstitute the lyophilized powder with 10 ml of normal saline.
Mechanism of Action
Pantoprazole irreversibly binds to the H+/K+-ATPase enzyme (proton pump) in the gastric parietal cells, inhibiting the final step of acid secretion. This leads to a dose-dependent inhibition of both basal and stimulated gastric acid secretion.

Excretion
Mainly excreted renally (approx. 80%) and in feces (approx. 20%).
Half life
Approximately 1 hour (terminal half-life), but the duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed after intravenous administration, bioavailability is nearly 100%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole or any component of the formulation.
- Co-administration with atazanavir or nelfinavir due to reduced antiviral efficacy.
Drug Interactions
Warfarin
Increased INR and prothrombin time, monitor carefully.
Methotrexate
Increased methotrexate levels, especially with high doses.
Ketoconazole, Itraconazole, Posaconazole, Erlotinib
Absorption of these drugs is pH-dependent and may be reduced by pantoprazole.
Storage
Store below 25°C, protect from light. Do not freeze. Reconstituted solution should be used within 2-4 hours.
