Description
Overview of the medicine
Omeprazole is a proton pump inhibitor used to treat conditions caused by excess stomach acid, such as GERD, ulcers, and Zollinger-Ellison Syndrome.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Duodenal ulcer: 20 mg once daily for 2-4 weeks. Gastric ulcer: 20 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial dose 60 mg once daily, adjusted based on response.
Elderly
No specific dosage adjustment, but caution is advised due to potential age-related decrease in liver function. Monitor for adverse effects.
Renal_impairment
No dosage adjustment required for renal impairment.
How to Take
Take the capsule whole with water, preferably in the morning before a meal. Do not chew, crush, or open the capsule.
Mechanism of Action
Omeprazole irreversibly binds to and inhibits the H+/K+-ATPase pump (proton pump) in gastric parietal cells, thereby blocking the final step in acid secretion.

Within 1-2 hours
Excretion
Mainly excreted in urine (approximately 77%) and feces (approximately 18%) as metabolites.
Half life
0.5-1 hour (but duration of acid suppression is much longer)
Absorption
Rapidly absorbed after oral administration. Bioavailability is about 30-40% but increases to 60% with repeated dosing.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Warfarin
Increased INR and risk of bleeding.
Clopidogrel
Reduced antiplatelet effect of clopidogrel (avoid concomitant use).
Methotrexate
Increased methotrexate levels, especially with high-dose methotrexate.
Atazanavir, Nelfinavir
Reduced absorption of these antivirals (contraindicated with nelfinavir).
Diazepam, Phenytoin, Citalopram
Omeprazole inhibits CYP2C19, potentially increasing plasma concentrations of these drugs.

Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are typically mild and may include nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Omeprazole is excreted in breast milk; use with caution in nursing mothers.
