Description
Overview of the medicine
Propin 20 mg Capsule is used to treat high blood pressure, angina, irregular heartbeats, and to prevent migraine headaches. It belongs to a group of medicines called beta-blockers.
Uses & Indications
Dosage
Adults
For hypertension: Initially 40 mg twice daily, may be increased gradually. For angina/arrhythmias/migraine: 10-40 mg 2-4 times daily.
Elderly
Lower initial doses may be appropriate; titrate slowly.
Renal_impairment
No significant dosage adjustment usually needed, but monitor carefully. Use with caution in severe renal impairment.
How to Take
Take orally, preferably before meals or on an empty stomach for better absorption, but consistently with food if stomach upset occurs. Do not crush or chew capsules.
Mechanism of Action
Propranolol is a non-selective beta-adrenergic receptor blocker. It blocks beta-1 and beta-2 adrenergic receptors, leading to decreased heart rate, myocardial contractility, and cardiac output, thereby lowering blood pressure. It also reduces renin release and has anti-arrhythmic effects.
Pharmacokinetics
Onset
Oral: 30-60 minutes
Excretion
Mainly via urine as metabolites, with a small amount of unchanged drug.
Half life
3-6 hours (immediate release)
Absorption
Rapidly and almost completely absorbed from the GI tract. Subject to significant first-pass metabolism.
Metabolism
Extensive hepatic metabolism, primarily by glucuronidation and hydroxylation.
Side Effects
Contraindications
- Bronchial asthma or bronchospasm
- Cardiogenic shock
- Sinus bradycardia
- Second or third-degree AV block
- Decompensated heart failure
- Prinzmetal's angina
Drug Interactions
NSAIDs (e.g., Ibuprofen)
May reduce the hypotensive effects of propranolol.
Insulin and oral hypoglycemics
Propranolol may mask symptoms of hypoglycemia and potentiate the hypoglycemic effect.
Alpha-blockers (e.g., Prazosin)
Increased risk of first-dose hypotension.
Calcium Channel Blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia and hypotension.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe bradycardia, hypotension, heart failure, bronchospasm, and hypoglycemia. Management involves supportive measures, atropine for bradycardia, glucagon, and vasopressors for hypotension.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Excreted in breast milk, use with caution during lactation.
