Description
Overview of the medicine
Prosalic Scalp Lotion is a combination medicine containing a potent corticosteroid (Betamethasone dipropionate) and a keratolytic agent (Salicylic Acid). It is used to treat inflammatory skin conditions on the scalp where hyperkeratosis (thickening of the skin) and scaling are prominent features, such as psoriasis or chronic eczema.
Uses & Indications
Dosage
Adults
Apply a few drops of the lotion to the affected scalp area twice daily (morning and night). Gently massage into the scalp. Treatment should be limited to 2-4 weeks, as directed by a physician.
Elderly
No specific dosage adjustment is generally required for the elderly, but caution should be exercised due to potentially thinner skin and increased risk of systemic absorption. Use under medical supervision.
Renal_impairment
No specific dosage adjustment is recommended for topical use; however, caution is advised with extensive application due to potential for systemic absorption of salicylic acid, especially in patients with severe renal impairment.
How to Take
For external use on the scalp only. Apply a thin film of the lotion to the affected area and rub it gently and completely into the scalp. Do not use occlusive dressings unless specifically advised by your doctor. Avoid contact with eyes, face, and mucous membranes. Wash hands thoroughly after application.
Mechanism of Action
Betamethasone dipropionate is a potent corticosteroid that exerts anti-inflammatory, antipruritic, and vasoconstrictive actions. It reduces inflammation by inhibiting the release of lysosomal enzymes, preventing macrophage accumulation in inflamed areas, reducing leukocyte adhesion, and inhibiting prostaglandin and leukotriene synthesis. Salicylic acid acts as a keratolytic agent by softening keratin, loosening the stratum corneum, and promoting the shedding of scales. This action also enhances the penetration of betamethasone.
Pharmacokinetics
Onset
Clinical effects are typically observed within hours to a few days of initiation of treatment.
Excretion
Betamethasone: Metabolites are primarily excreted in the urine. Salicylic acid: Excreted predominantly by the kidneys, primarily as metabolites.
Half life
Betamethasone: Systemic half-life is variable, approximately 5-8 hours. Salicylic acid: Systemic half-life is approximately 2-3 hours (topical application).
Absorption
Both betamethasone dipropionate and salicylic acid can be absorbed systemically through the skin. The extent of absorption depends on the area of application, duration of treatment, integrity of the skin barrier, and use of occlusive dressings. Increased absorption occurs with damaged skin or in children.
Metabolism
Betamethasone: Primarily metabolized in the liver to inactive metabolites. Salicylic acid: Metabolized in the liver to salicyluric acid and glucuronides.
Side Effects
Contraindications
- Hypersensitivity to betamethasone dipropionate, salicylic acid, or any component of the formulation
- Viral infections of the skin (e.g., herpes simplex, chickenpox, shingles)
- Fungal or bacterial skin infections not adequately treated concurrently
- Perioral dermatitis, rosacea, acne vulgaris
- Children under 2 years of age
Drug Interactions
Other topical corticosteroids
Concurrent use with other topical corticosteroids should be avoided due to increased risk of systemic adverse effects and local skin changes.
Other topical keratolytic agents
Concurrent use with other topical preparations containing keratolytic agents (e.g., urea, lactic acid) may increase the risk of local irritation and excessive desquamation.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Excessive or prolonged use of Prosalic Scalp Lotion can lead to systemic effects. Symptoms of corticosteroid overdose include HPA axis suppression, Cushing's syndrome manifestations, hyperglycemia, and glucosuria. Symptoms of salicylic acid overdose (salicylism) may include tinnitus, dizziness, headache, confusion, and gastrointestinal upset. In case of overdose, discontinue the medication gradually and seek medical attention. Symptomatic and supportive treatment should be initiated.
Pregnancy & Lactation
Topical corticosteroids and salicylates should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Avoid large areas, prolonged use, and occlusive dressings. It is not known whether topically administered corticosteroids or salicylic acid are excreted in human milk. Use with caution in nursing mothers and only if clearly needed. Consult a doctor.

Clinical Trials
Clinical efficacy of topical betamethasone dipropionate and salicylic acid combination products is well-established for corticosteroid-responsive dermatoses with hyperkeratosis, supported by numerous studies on the individual components and the combination. Specific large-scale, recent clinical trials for 'Prosalic Scalp Lotion' brand may vary by region, but the therapeutic principles are widely accepted.
Lab Monitoring
- Routine lab monitoring is not typically required for topical application. However, if there is a concern for systemic absorption due to extensive or prolonged use, periodic assessment of plasma cortisol levels may be considered to evaluate HPA axis suppression.
Doctor Notes
- Emphasize proper application technique and duration of treatment to minimize systemic absorption and local side effects.
- Advise patients to report any signs of skin irritation, systemic effects, or lack of improvement.
- Avoid use on the face, intertriginous areas, or broken skin due to increased absorption and risk of adverse reactions.
- Consider alternative treatments if the condition does not respond within the recommended treatment period.
Patient Guidelines
- Use this medication exactly as prescribed by your doctor. Do not use more or less, or for longer than recommended.
- This lotion is for external use on the scalp only. Do not apply it to the face, groin, or underarms unless specifically instructed by your doctor.
- Avoid contact with eyes, nose, mouth, and other mucous membranes. If contact occurs, rinse thoroughly with water.
- Do not apply to open wounds, broken skin, or areas of infection unless directed by your doctor.
- Wash your hands thoroughly before and after applying the lotion.
Missed Dose Advice
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not apply a double dose to make up for a missed one.
Driving Precautions
Prosalic Scalp Lotion is for topical use and is not expected to affect the ability to drive or operate machinery.
