Description
Overview of the medicine
Protemin-5-Injection is a sterile solution of essential and non-essential amino acids used for parenteral nutrition in patients unable to take sufficient oral nutrition or having protein deficiency.
Uses & Indications
Dosage
Adults
Typically 1-1.5 g amino acids/kg body weight/day, adjusted based on patient's nutritional status, metabolic needs, and clinical condition. Infusion rate should not exceed 0.1 g/kg/hour.
Elderly
Lower doses may be considered, starting at 0.8-1 g/kg/day, with careful monitoring due to potential renal and hepatic impairment.
Renal_impairment
Reduced dosage and careful monitoring of electrolyte and fluid balance, often 0.5-0.7 g/kg/day, or use of specialized renal formulations. Avoid in severe renal failure without dialysis.
How to Take
Administered via slow intravenous infusion, typically through a central venous catheter for long-term use, or a peripheral vein for shorter durations. Must be administered under strict medical supervision.
Mechanism of Action
Provides a balanced supply of amino acids necessary for protein synthesis, tissue repair, and overall metabolic support, bypassing the gastrointestinal tract and directly entering systemic circulation.
Pharmacokinetics
Onset
Immediate upon initiation of infusion.
Excretion
Metabolites (e.g., urea) are primarily excreted via kidneys. Unutilized amino acids may be excreted unchanged in urine.
Half life
Varies depending on individual metabolic rate and specific amino acids; generally utilized rapidly by tissues for protein synthesis or energy.
Absorption
Administered intravenously, hence 100% bioavailability into systemic circulation.
Metabolism
Amino acids are metabolized in various tissues, primarily the liver, into proteins, glucose, or energy, or converted to urea for excretion.
Side Effects
Contraindications
- •Severe liver disease (hepatic coma)
- •Severe kidney disease (renal failure without dialysis)
- •Severe metabolic acidosis
- •Unstable circulatory conditions (e.g., shock)
- •Known hypersensitivity to any component of the formulation
- •Inborn errors of amino acid metabolism
Drug Interactions
Insulin
Dosage of insulin may need adjustment as amino acids can affect glucose metabolism.
Tetracycline
May increase protein catabolism and nitrogen excretion. Not recommended for concomitant use.
Corticosteroids
May influence protein metabolism, requiring careful monitoring and potential dosage adjustments of amino acid solutions.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include fluid overload, electrolyte imbalance, metabolic acidosis, hyperaminoacidemia, and increased blood urea nitrogen. Management involves discontinuing infusion, supportive care, and correcting imbalances (e.g., fluid and electrolyte replacement, dialysis if severe renal impairment).
Pregnancy & Lactation
Use only if clearly needed and the potential benefits outweigh the risks to the fetus or infant. Consult a physician for individualized assessment and advice.
