Description
Overview of the medicine
Deflazacort is a glucocorticoid used as an anti-inflammatory and immunosuppressant agent. It is used to treat various conditions such as Duchenne muscular dystrophy, asthma, arthritis, and allergic reactions. It has a slightly better side-effect profile compared to some other corticosteroids.
Uses & Indications
Dosage
Adults
Dosage varies greatly depending on the condition and patient response. Typically, initial doses range from 6 mg to 90 mg daily, adjusted downwards for maintenance. For Duchenne muscular dystrophy, 0.9 mg/kg/day once daily.
Elderly
No specific dose adjustment for elderly patients is usually required, but caution is advised due to potential age-related organ function decline.
Renal_impairment
No specific dose adjustment is generally needed for renal impairment, but caution is advised.
How to Take
Take orally, with or without food. It is recommended to take it in the morning to align with the body's natural corticosteroid rhythm. Do not stop abruptly without consulting a doctor.
Mechanism of Action
Deflazacort binds to glucocorticoid receptors in the cytoplasm, forming a complex that translocates to the nucleus. This complex modulates gene expression, leading to the synthesis of anti-inflammatory proteins and inhibition of pro-inflammatory mediators. It also reduces immune cell activity.
Pharmacokinetics
Onset
Within a few hours.
Excretion
Mainly excreted via the kidneys (urine) as inactive metabolites.
Half life
Active metabolite half-life is approximately 1.5-2 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract and converted to its active metabolite, 21-desacetyl deflazacort.
Metabolism
Extensively metabolized in the liver to the active metabolite (21-desacetyl deflazacort) and then to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to deflazacort or any components of the tablet.
- Systemic fungal infections.
- Administration of live or live-attenuated vaccines during therapy.
Drug Interactions
NSAIDs
Increased risk of gastrointestinal bleeding/ulceration.
Diuretics
Increased risk of hypokalemia.
Ketoconazole, Erythromycin
May increase deflazacort's effects by inhibiting its metabolism.
Phenytoin, Phenobarbital, Rifampicin
May reduce deflazacort's effects by increasing its metabolism.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely to cause life-threatening effects. Chronic overdose can lead to signs of hypercorticism (Cushing's syndrome). Treatment is symptomatic and supportive; gradual withdrawal if chronic overdose.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Small amounts may pass into breast milk; caution is advised during breastfeeding.
