Overview of the medicine
Pylotrip-R 20 mg Tablet contains Rabeprazole, a proton pump inhibitor, used to reduce the amount of acid produced in the stomach. It treats conditions like heartburn, acid reflux, stomach ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily in combination with antibiotics for 7-14 days. Zollinger-Ellison: Initial 60 mg once daily, adjusted as needed.
Elderly
No dose adjustment generally required, but caution with severe hepatic impairment.
Renal_impairment
No dose adjustment required for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Pylotrip-R should be swallowed whole with water before a meal (preferably in the morning). Do not chew or crush the tablet.
Mechanism of Action
Rabeprazole is a proton pump inhibitor that irreversibly blocks the H+/K+-ATPase enzyme system (proton pump) in the gastric parietal cells. This action effectively reduces basal and stimulated gastric acid secretion.

Metabolism
Extensively metabolized in the liver by cytochrome P450 (CYP3A4 and CYP2C19) to inactive metabolites.

Contraindications
- Hypersensitivity to rabeprazole or any components of the formulation.
- Hypersensitivity to other benzimidazole substituted proton pump inhibitors.
Drug Interactions
Digoxin
Increased digoxin exposure due to increased gastric pH.
Warfarin
Increased INR and prothrombin time reported, monitor closely.
Methotrexate
Increased and prolonged methotrexate serum levels and/or its metabolite.
Atazanavir/Nelfinavir
Coadministration with PPIs may significantly reduce plasma concentrations of atazanavir/nelfinavir, leading to loss of antiviral effect.

Overdose
Limited experience with overdose. Symptoms may include confusion, blurred vision, tachycardia, nausea, sweating, headache, and dry mouth. Management is symptomatic and supportive.
