Description
Overview of the medicine
Pyralgin 120 mg Suspension is an oral medication containing Paracetamol, used to relieve mild to moderate pain and reduce fever in children. It works by blocking the production of certain chemical messengers in the brain that cause pain and fever.
Uses & Indications
Dosage
Adults
Not typically used for adults in this strength. Adults usually take 500 mg or 665 mg tablets.
Elderly
Not typically used for elderly in this strength. Adults usually take 500 mg or 665 mg tablets.
Renal_impairment
In severe renal impairment, dose interval should be extended (e.g., every 8 hours). Consult a physician.
How to Take
For oral administration. Shake the bottle well before use. Measure the prescribed dose using the provided measuring cup or syringe. Can be taken with or without food.
Mechanism of Action
Paracetamol (Acetaminophen) is thought to act primarily in the central nervous system by inhibiting prostaglandin synthesis, particularly COX-3, which results in its analgesic and antipyretic effects. It has weak anti-inflammatory activity.
Pharmacokinetics
Onset
Analgesia: 30-60 minutes; Antipyresis: within 30 minutes.
Excretion
Mainly via urine as glucuronide and sulfate conjugates; less than 5% excreted unchanged.
Half life
1.5 to 3 hours (increased in hepatic dysfunction or overdose).
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30-60 minutes.
Metabolism
Primarily hepatic, conjugated with glucuronic acid and sulfate. A minor pathway involves cytochrome P450, producing a potentially hepatotoxic intermediate (NAPQI) normally detoxified by glutathione.
Side Effects
Contraindications
- Known hypersensitivity to Paracetamol or any other component of the formulation.
- Severe hepatic impairment or active liver disease.
Drug Interactions
Alcohol
Chronic alcohol consumption may increase the risk of hepatotoxicity.
Warfarin
May increase the anticoagulant effect of warfarin, increasing the risk of bleeding, especially with prolonged, high-dose use.
Cholestyramine
May decrease the absorption of paracetamol.
Metoclopramide, Domperidone
May increase the absorption rate of paracetamol.
Phenobarbital, Phenytoin, Carbamazepine, Rifampicin
May accelerate paracetamol metabolism, potentially increasing the risk of hepatotoxicity.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Overdose can cause severe, potentially fatal, hepatic necrosis. Symptoms include nausea, vomiting, abdominal pain, sweating, and malaise, often appearing 24-48 hours after ingestion. Management involves gastric lavage, activated charcoal (if within 1 hour of ingestion), and administration of N-acetylcysteine as an antidote.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation at recommended doses, but consult a doctor or pharmacist before use.

YesClinical Trials
Paracetamol is an extensively studied and well-established drug. Most clinical trials focus on new formulations, combinations, or specific populations rather than its basic efficacy for fever and pain.
Lab Monitoring
- Liver function tests (in cases of overdose, prolonged use, or suspected hepatotoxicity)
- Blood counts (rarely, in cases of suspected blood dyscrasias)
Doctor Notes
- Educate parents/caregivers on proper dosing based on weight and age, and the importance of using accurate measuring devices.
- Caution against concomitant use of other paracetamol-containing products to prevent overdose.
- Advise on symptoms of overdose and when to seek immediate medical attention.
- Monitor liver function in patients with pre-existing liver conditions or those on long-term therapy.
Patient Guidelines
- Always follow the prescribed dose and frequency.
- Do not exceed the maximum daily dose (usually 60 mg/kg/day, up to 4 doses in 24 hours, or as directed by a physician).
- Check other medications to ensure they do not also contain paracetamol to avoid overdose.
- Consult a doctor if fever or pain persists for more than 3 days, or if new symptoms appear.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose.
Driving Precautions
Paracetamol is generally not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
- Ensure adequate fluid intake, especially during fever.
- Get sufficient rest.
