Description
Overview of the medicine
Pyrin 500 mg tablet contains Paracetamol, a widely used analgesic and antipyretic medicine for the relief of mild to moderate pain and fever.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg every 4-6 hours as needed. Maximum 4000 mg (4g) in 24 hours.
Elderly
No specific dose adjustment required, but caution is advised in patients with impaired liver or kidney function.
Renal_impairment
Extended dosing interval may be necessary in severe renal impairment (CrCl <10 mL/min). Consult physician.
How to Take
Oral. Can be taken with or without food. Do not exceed the recommended dose.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system, inhibiting prostaglandin synthesis by inhibiting cyclooxygenase (COX) enzymes, mainly COX-2, leading to analgesic and antipyretic effects. It has weak anti-inflammatory properties.
Pharmacokinetics
Onset
30-60 minutes for analgesia, 30 minutes for antipyresis.
Excretion
Excreted mainly in the urine as glucuronide and sulfate conjugates, with less than 5% excreted as unchanged drug.
Half life
1.5 to 3 hours in adults.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30-60 minutes.
Metabolism
Primarily metabolized in the liver, mainly by conjugation with glucuronic acid and sulfate. A small portion is metabolized by cytochrome P450 into a highly reactive intermediate (NAPQI), which is detoxified by glutathione.
Side Effects
Contraindications
- Hypersensitivity to paracetamol or any component of the formulation
- Severe hepatic impairment or active liver disease
Drug Interactions
Alcohol
Increased risk of liver damage, especially with chronic heavy alcohol consumption.
Warfarin
May enhance anticoagulant effect, increasing risk of bleeding, especially with prolonged daily use.
Isoniazid, Rifampicin, Carbamazepine, Phenytoin
May increase risk of hepatotoxicity due to increased metabolism of paracetamol to toxic metabolites.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, abdominal pain, anorexia, and sweating. Severe overdose can lead to hepatic necrosis. Treatment involves gastric lavage, activated charcoal, and administration of N-acetylcysteine (NAC) as an antidote.
