Description
Overview of the medicine
Quetiapine is an atypical antipsychotic used to treat schizophrenia, bipolar disorder (manic, depressive, and mixed episodes), and as an adjunctive treatment for major depressive disorder. Queta 25 mg tablets contain 25 mg of Quetiapine fumarate.
Uses & Indications
Dosage
Adults
Starting dose for schizophrenia often 25 mg twice daily, titrated up to 150-750 mg/day. For bipolar mania, initial dose 50 mg twice daily, titrated up to 400-800 mg/day. For bipolar depression, 50 mg at bedtime, titrated up to 300 mg at bedtime. For MDD adjunctive, 50 mg at bedtime, titrated up to 300 mg at bedtime.
Elderly
Lower starting dose (e.g., 25 mg/day) and slower titration recommended due to increased sensitivity and higher incidence of adverse effects.
Renal_impairment
No dosage adjustment necessary for renal impairment.
Hepatic_impairment
Initial dose of 25 mg/day recommended, with careful titration.
How to Take
Oral administration. Queta IR tablets can be taken with or without food. XR tablets are usually taken once daily at night, without food or a light meal.
Mechanism of Action
Quetiapine acts as an antagonist at multiple neurotransmitter receptors. It exhibits high affinity for serotonin (5-HT2A) and dopamine (D1 and D2) receptors, as well as histamine (H1) and alpha1-adrenergic receptors. Its antipsychotic and mood-stabilizing effects are primarily attributed to its antagonism of 5-HT2A and D2 receptors.
Pharmacokinetics
Onset
Acute sedation and anxiolysis may occur within hours. Therapeutic effects for psychosis or mood stabilization develop over several days to weeks.
Excretion
Approximately 73% excreted in urine and 20% in feces, predominantly as metabolites.
Half life
Approximately 6-7 hours for quetiapine and ~12 hours for its active metabolite, norquetiapine.
Absorption
Rapidly and well absorbed after oral administration. Peak plasma concentrations reached within 1.5 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, to inactive and active metabolites, including norquetiapine.
Side Effects
Contraindications
- Known hypersensitivity to quetiapine or any component of the formulation.
Drug Interactions
Anticholinergic drugs
Increased risk of anticholinergic side effects.
CYP3A4 Inducers (e.g., carbamazepine, rifampin)
May decrease quetiapine plasma levels, requiring dose increase.
CNS Depressants (e.g., alcohol, benzodiazepines)
May potentiate sedative effects.
CYP3A4 Inhibitors (e.g., ketoconazole, erythromycin)
May increase quetiapine plasma levels, requiring dose reduction.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms. Management is supportive, focusing on maintaining an open airway, ensuring adequate oxygenation and ventilation, and managing cardiovascular support. Gastric lavage and activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, breastfeeding is not recommended.
