Description
Overview of the medicine
Queta-XR 200 mg Tablet is an atypical antipsychotic medicine used to treat conditions such as schizophrenia, bipolar disorder (manic or depressive episodes), and as an add-on treatment for major depressive disorder.
Uses & Indications
Dosage
Adults
Dosage varies by indication. For major depressive disorder (adjunctive), usually 50-300 mg/day. For schizophrenia or bipolar mania, usually 400-800 mg/day. Administer once daily in the evening.
Elderly
Lower starting dose (e.g., 50 mg/day) and slower titration, usually 50 mg/day increments, depending on clinical response and tolerability.
Renal_impairment
No dose adjustment is usually required in patients with renal impairment.
How to Take
Take orally once daily, preferably in the evening, with or without food. Swallow the tablet whole; do not chew, crush, or split it.
Mechanism of Action
Quetiapine works by antagonizing a range of neurotransmitter receptors, including serotonin 5-HT2A and dopamine D2 receptors, contributing to its antipsychotic, antidepressant, and mood-stabilizing effects.
Pharmacokinetics
Onset
Antipsychotic effects may take 1-3 weeks for full manifestation. Sedation may occur earlier.
Excretion
Approximately 73% excreted in urine and 20% in feces within 120 hours, predominantly as metabolites.
Half life
The apparent elimination half-life of quetiapine is approximately 7 hours.
Absorption
Well absorbed. Peak plasma concentration (Tmax) is approximately 6 hours after administration of the extended-release tablet.
Metabolism
Primarily hepatic, largely via the cytochrome P450 3A4 (CYP3A4) enzyme system.
Side Effects
Contraindications
- Hypersensitivity to quetiapine or any component of the formulation.
- Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin, indinavir, nefazodone, nelfinavir, ritonavir, saquinavir) is contraindicated.
Drug Interactions
CNS Depressants
May potentiate the effects of alcohol and other CNS depressants.
CYP3A4 Inducers
Concomitant use may significantly decrease quetiapine plasma concentrations, necessitating a higher dose (e.g., carbamazepine, phenytoin, rifampin).
Antihypertensives
May potentiate orthostatic hypotension.
CYP3A4 Inhibitors
Concomitant use may significantly increase quetiapine plasma concentrations, necessitating a dose reduction (e.g., ketoconazole, clarithromycin).
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, sedation, tachycardia, and hypotension. Management involves supportive measures, maintaining an adequate airway, oxygenation, and ventilation, and monitoring cardiovascular function.
Pregnancy & Lactation
Pregnancy Category C: Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as quetiapine is excreted in human milk.
