Overview of the medicine
Quetiapine is an atypical antipsychotic used to treat schizophrenia, bipolar disorder, and as an adjunct for major depressive disorder. Quetinil 25 mg is a low-dose tablet often used for initial treatment or specific indications.
Uses & Indications
Dosage
Adults
Initial dose typically 25 mg twice daily for schizophrenia/bipolar mania. For bipolar depression, 50 mg at bedtime. Doses are titrated gradually based on clinical response and tolerability, up to a range of 300-800 mg/day depending on indication.
Elderly
Lower starting dose (e.g., 25 mg once daily) and slower titration recommended due to increased sensitivity and potential for adverse effects.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment, but caution is advised in severe cases.
How to Take
Take orally, with or without food. For some indications, it may be taken at bedtime.
Mechanism of Action
Quetiapine is an antagonist at multiple neurotransmitter receptors. It acts as an antagonist at serotonin 5-HT2 and dopamine D1 and D2 receptors. It also has antagonistic effects on histamine H1 and adrenergic alpha1 and alpha2 receptors. The combination of D2 and 5-HT2 antagonism contributes to its therapeutic effects.

Onset
Antipsychotic effects may take days to weeks to manifest fully. Sedative effects can be seen sooner.
Excretion
Approximately 73% excreted in urine and 20% in feces within 120 hours, mainly as metabolites.
Half life
Approximately 6-7 hours for quetiapine and about 12 hours for its active metabolite norquetiapine.
Absorption
Rapidly absorbed after oral administration, reaching peak plasma concentrations in approximately 1.5 hours.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 3A4 (CYP3A4) enzyme. Its main active metabolite is norquetiapine.

Contraindications
- •Hypersensitivity to quetiapine or any component of the formulation.
- •Concomitant use with strong cytochrome P450 3A4 (CYP3A4) inhibitors (e.g., ketoconazole, erythromycin, clarithromycin, nefazodone, protease inhibitors).
Drug Interactions
Antihypertensives
May enhance hypotensive effects due to quetiapine's alpha1-adrenergic antagonism.
CYP3A4 inhibitors (e.g., ketoconazole, fluconazole)
May significantly increase quetiapine plasma concentrations, requiring dose reduction.
CNS depressants (e.g., alcohol, benzodiazepines, opioids)
May have additive CNS depressant effects, increasing sedation and respiratory depression.
CYP3A4 inducers (e.g., carbamazepine, phenytoin, rifampicin)
May significantly decrease quetiapine plasma concentrations, requiring dose increase.
Storage

Overdose
Symptoms of overdose include somnolence, sedation, tachycardia, hypotension, and anticholinergic effects. Management is primarily supportive, including maintaining an open airway, ensuring adequate oxygenation and ventilation, and monitoring cardiovascular status.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as quetiapine is excreted into breast milk.
