Overview of the medicine
Quilev 500 mg Tablet is an antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, urinary tract infections, and prostate infections. It belongs to the fluoroquinolone class of antibiotics.
Uses & Indications
Dosage
Adults
Generally 500 mg orally once daily for 7-14 days, depending on the infection.
Elderly
No specific dose adjustment is generally required, but dose reduction may be necessary in patients with impaired renal function.
Renal_impairment
Dosage adjustment is necessary based on creatinine clearance. For CrCl 20-49 mL/min, initial dose 500 mg, then 250 mg every 24 hours.
How to Take
Take orally with or without food. Ensure adequate hydration. Do not take with antacids containing magnesium or aluminum, sucralfate, didanosine (chewable/buffered tablets or pediatric powder), or metal cations (e.g., iron, zinc).
Mechanism of Action
Levofloxacin works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.

Side Effects
Contraindications
- Hypersensitivity to levofloxacin or other fluoroquinolones.

Drug Interactions
NSAIDs
May increase the risk of CNS stimulation and convulsive seizures when co-administered with fluoroquinolones.
Warfarin
May enhance the anticoagulant effects of warfarin, increasing the risk of bleeding. Monitor INR closely.
Corticosteroids
Increased risk of tendinitis and tendon rupture when co-administered with fluoroquinolones.
Antacids, Sucralfate, Metal Cations
Can significantly reduce oral bioavailability of levofloxacin. Administer levofloxacin at least 2 hours before or 2 hours after these agents.
Storage
Store below 30°C in a dry place, protected from light and moisture.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. Gastric lavage may be considered. Monitor ECG for QT prolongation.
Pregnancy & Lactation
Not recommended during pregnancy or lactation unless the potential benefits outweigh the risks. Animal studies indicate potential harm to fetal cartilage. Levofloxacin is excreted in human milk.
