Description
Overview of the medicine
Qutap 100 mg Tablet contains Quetiapine Fumarate, an atypical antipsychotic medication used for the treatment of schizophrenia, bipolar disorder (manic, depressive, and maintenance episodes), and as an adjunctive treatment for major depressive disorder.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 25-50 mg twice daily, titrated to 300-450 mg/day in 2-3 divided doses. Max 750 mg/day. Bipolar Mania: Initially 100 mg on Day 1, 200 mg on Day 2, then titrate to 400-800 mg/day. Bipolar Depression: 50 mg at bedtime, titrate to 300 mg at bedtime.
Elderly
Initiate with 25 mg/day, increasing by 25-50 mg/day depending on clinical response and tolerability. Slower titration is recommended.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment, initiate with 25 mg/day and titrate slowly.
How to Take
Qutap tablet should be taken orally, with or without food. It should be swallowed whole and not crushed or chewed.
Mechanism of Action
Quetiapine's therapeutic effects are mediated through a combination of antagonism at serotonin (5-HT2A) and dopamine (D1 and D2) receptors. It also has affinity for histamine H1 and adrenergic α1/α2 receptors, contributing to its sedative and orthostatic hypotensive effects.
Pharmacokinetics
Onset
Sedative effects may begin within hours. Antipsychotic effects typically take several weeks to fully manifest.
Excretion
Approximately 73% excreted in the urine and 20% in the feces within 5 days, mainly as metabolites.
Half life
Approximately 6-7 hours for quetiapine; the active metabolite norquetiapine has a half-life of about 12 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, with peak plasma concentrations occurring approximately 1.5 hours after oral administration.
Metabolism
Extensively metabolized in the liver, primarily via cytochrome P450 3A4 (CYP3A4) enzyme system, to active (norquetiapine) and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to quetiapine or any component of the tablet.
- Concomitant use with strong cytochrome P450 3A4 (CYP3A4) inhibitors (e.g., ketoconazole, erythromycin, protease inhibitors).
Drug Interactions
CNS Depressants
Additive effects with alcohol and other CNS depressants. Avoid concomitant use.
CYP3A4 Inducers
May decrease quetiapine plasma concentrations (e.g., phenytoin, carbamazepine, rifampicin). Dose increase of quetiapine may be necessary.
Antihypertensives
May enhance orthostatic hypotension effects.
CYP3A4 Inhibitors
May increase quetiapine plasma concentrations (e.g., ketoconazole, erythromycin). Dose reduction of quetiapine may be necessary.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms. Management is primarily supportive, involving maintaining an open airway, ensuring adequate oxygenation and ventilation, and monitoring cardiovascular function. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in human milk; avoid breastfeeding during treatment.
