Description
Overview of the medicine
Rabegend 20 mg Capsule is a proton pump inhibitor (PPI) used to treat conditions caused by excess stomach acid, such as heartburn, acid reflux, and peptic ulcers.
Uses & Indications
Dosage
Adults
Typically 20 mg once daily, usually in the morning before food. Duration varies based on condition (e.g., 4-8 weeks for GERD/ulcers, 7-14 days for H. pylori eradication with antibiotics). Maintenance dose may be 10 mg or 20 mg daily.
Elderly
No dosage adjustment is generally required for elderly patients, but caution is advised due to potential for reduced hepatic function.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Rabegend 20 mg Capsule should be swallowed whole with water, typically in the morning before breakfast. Do not crush or chew the capsule.
Mechanism of Action
Rabeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby blocking the final step of acid secretion in the stomach. This leads to a sustained reduction in gastric acid production.
Pharmacokinetics
Onset
Onset of action within 1 hour; maximum acid suppression within 2-4 hours.
Excretion
Approximately 90% of the drug is excreted in the urine and 10% in the feces as metabolites.
Half life
Plasma elimination half-life is approximately 1-2 hours.
Absorption
Rapidly absorbed after oral administration; bioavailability is about 52%. Food has no significant effect on the extent of absorption.
Metabolism
Extensively metabolized in the liver primarily by CYP2C19 and CYP3A4 into inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to rabeprazole, substituted benzimidazoles (e.g., other PPIs), or any component of the formulation.
- Known allergy to this class of drugs.
Drug Interactions
Digoxin
Increased systemic exposure to digoxin.
Warfarin
Increased INR and prothrombin time; monitor closely.
Clopidogrel
Reduced antiplatelet activity of clopidogrel.
Methotrexate
Increased and prolonged elevation of methotrexate levels; dose adjustment or temporary discontinuation may be needed.
Atazanavir/Nelfinavir
Reduced plasma concentrations and therapeutic effect due to decreased gastric pH; co-administration is not recommended or requires careful monitoring.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for rabeprazole overdose. Management should be symptomatic and supportive. Rabeprazole is extensively protein bound and is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Rabeprazole is excreted in breast milk; therefore, caution should be exercised when administered to a nursing woman.
