Description
Overview of the medicine
Rabenta (Rabeprazole) is a proton pump inhibitor (PPI) used to treat conditions caused by excess stomach acid, such as heartburn, gastroesophageal reflux disease (GERD), and peptic ulcers.
Uses & Indications
Dosage
Adults
Typically 20 mg once daily, usually in the morning before breakfast. May be increased to 40 mg daily for severe conditions or up to 60 mg twice daily for Zollinger-Ellison Syndrome.
Elderly
No dosage adjustment generally required, but caution is advised.
Renal_impairment
No dosage adjustment necessary for mild to moderate renal impairment. Caution with severe impairment.
How to Take
Take orally, swallowed whole with water. Do not chew or crush. Best taken before breakfast.
Mechanism of Action
Rabeprazole inhibits the H+/K+-ATPase enzyme system (the 'proton pump') at the secretory surface of the gastric parietal cell, thereby blocking the final step of acid production.
Pharmacokinetics
Onset
Within 1 hour; maximal effect within 2-4 hours.
Excretion
Primarily excreted in urine (approximately 90%) and feces (approximately 10%).
Half life
Approximately 1-2 hours.
Absorption
Rapidly absorbed after oral administration; bioavailability is about 52% and increases with repeated dosing.
Metabolism
Extensively metabolized in the liver via cytochrome P450 (CYP2C19 and CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to rabeprazole or any proton pump inhibitor
- Known allergic reaction to the drug components
Drug Interactions
Warfarin
Increased INR/prothrombin time.
Clopidogrel
Potential reduction in antiplatelet effect (less significant than with omeprazole).
Methotrexate
Increased plasma concentration of methotrexate.
Ketoconazole, Itraconazole
Reduced absorption due to altered gastric pH.
Storage
Store in a cool, dry place below 30°C, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include confusion, headache, drowsiness, blurred vision, fast heartbeat, nausea, vomiting, sweating, and dry mouth. Management is symptomatic and supportive; hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Avoid during breastfeeding as rabeprazole is excreted in milk.
