Description
Overview of the medicine
Rabeprazole is a proton pump inhibitor (PPI) used to treat conditions involving excessive stomach acid production, such as gastroesophageal reflux disease (GERD), peptic ulcer disease, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Typically 20 mg once daily, taken before a meal. Duration of treatment varies depending on the condition.
Elderly
No specific dose adjustment is required for elderly patients.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Take the tablet whole with a glass of water, preferably in the morning before breakfast. Do not chew, crush, or break the tablet.
Mechanism of Action
Rabeprazole irreversibly blocks the H+/K+ ATPase enzyme system (the proton pump) in the gastric parietal cells. This action significantly reduces both basal and stimulated gastric acid secretion.

Onset
Within 1 hour of administration.
Excretion
Excreted predominantly in the urine (around 90%) and feces (around 10%) as metabolites.
Half life
Approximately 1-2 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 2-4 hours. Bioavailability is approximately 52%.
Metabolism
Extensively metabolized in the liver primarily via cytochrome P450 (CYP2C19 and CYP3A4) isoenzymes.
Side Effects
Contraindications
- Hypersensitivity to rabeprazole, substituted benzimidazoles, or any component of the formulation.
- Patients with known gastrointestinal malignancy (until excluded).
Drug Interactions
Digoxin
Increased absorption and plasma concentration of digoxin.
Warfarin
Increased INR and prothrombin time, monitor carefully.
Methotrexate
Increased plasma levels of methotrexate, especially with high-dose therapy.
Drugs dependent on gastric pH
Ketoconazole, Itraconazole, Atazanavir, Nelfinavir (absorption may be altered due to increased gastric pH).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is limited experience with overdose. In case of overdose, provide symptomatic and supportive treatment. Rabeprazole is not readily dialyzable.
Pregnancy & Lactation
Use in pregnancy only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Not recommended during breastfeeding as it is excreted in breast milk.
