Description
Overview of the medicine
Rabeprazole is a proton pump inhibitor (PPI) used to treat conditions caused by too much stomach acid, such as heartburn, stomach ulcers, and gastroesophageal reflux disease (GERD).
Uses & Indications
Dosage
Adults
GERD: 20 mg once daily for 4-8 weeks. Duodenal ulcer: 20 mg once daily for 4 weeks. Gastric ulcer: 20 mg once daily for 6 weeks. Zollinger-Ellison Syndrome: Initial 60 mg once daily, adjusted as needed.
Elderly
No dosage adjustment generally required, but caution in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment required.
How to Take
Should be taken orally, preferably in the morning before breakfast. Swallow the tablet whole; do not crush, chew, or split it.
Mechanism of Action
Rabeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme (proton pump) in the gastric parietal cells. This reduces the secretion of gastric acid.

Excreted primarily via urine (approximately 90%) and feces.
Half life
Approximately 1 to 2 hours
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 52% and is not significantly affected by food.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to rabeprazole, other substituted benzimidazoles, or any component of the formulation.
- Patients taking atazanavir or nelfinavir due to potential reduction in antiretroviral efficacy.
Drug Interactions
Digoxin
Increased bioavailability of digoxin.
Warfarin
Monitor INR; increased risk of bleeding.
Methotrexate
Increased methotrexate levels, especially with high doses.
Ketoconazole/Itraconazole
Decreased absorption of these drugs due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Treatment is symptomatic and supportive. Rabeprazole is not readily dialyzable.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefit outweighs the risk to the fetus. It is not known whether rabeprazole is excreted in human milk; use with caution in breastfeeding mothers.
