Description
Overview of the medicine
Radex 5 mg Tablet is a proton pump inhibitor (PPI) used to reduce stomach acid production. It is prescribed for conditions like gastroesophageal reflux disease (GERD), stomach ulcers, and other acid-related digestive problems. The 5mg strength might be used for milder conditions or maintenance therapy.
Uses & Indications
Dosage
Adults
Typically 5 mg once daily, preferably in the morning before breakfast, or as directed by the physician. Dosage may be adjusted based on clinical response.
Elderly
No specific dosage adjustment is generally required for elderly patients, but use with caution.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Take the tablet whole with a glass of water, preferably in the morning before food. Do not chew or crush the tablet.
Mechanism of Action
Rabeprazole selectively and irreversibly inhibits the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cells. This action blocks the final step of acid production, leading to a sustained decrease in gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Mainly excreted in urine (approximately 90%) as metabolites, with a small amount in feces.
Half life
Approximately 1-2 hours (for parent compound), but acid suppression effect is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration; bioavailability is about 52% (for 20mg dose), increasing with repeated dosing. Absorption is not significantly affected by food.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to rabeprazole, substituted benzimidazoles, or any component of the formulation.
- •Children below 12 years (unless specifically indicated and supervised).
Drug Interactions
Digoxin
Increased plasma levels of digoxin due to increased gastric pH.
Warfarin
Possible increased INR/prothrombin time; monitor closely.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to CYP2C19 inhibition.
Methotrexate
Increased plasma levels of methotrexate, especially with high doses.
Ketoconazole/Itraconazole
Reduced absorption of these drugs due to increased gastric pH.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
There is limited experience with overdose. Symptoms may include drowsiness, tachycardia, headache, and nausea. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. It is unknown if rabeprazole is excreted in human milk; use with caution in nursing mothers.
