Description
Overview of the medicine
Recof-15 mg Syrup is an antitussive medication used for the symptomatic relief of dry, unproductive coughs by suppressing the cough reflex.
Uses & Indications
Dosage
Adults
10-20 mg (5-10 ml) every 4-6 hours as needed, not to exceed 120 mg (60 ml) in 24 hours.
Elderly
Similar to adult dosage, but use with caution due to potential for increased sensitivity.
Renal_impairment
Use with caution. Dosage adjustment may be necessary in severe renal impairment. Consult a physician.
How to Take
Administer orally, with or without food. Use a measuring device for accurate dosing.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla oblongata to elevate the cough threshold, thereby suppressing the cough reflex.
Pharmacokinetics
Onset
15-30 minutes
Excretion
Mainly excreted by the kidneys as unchanged drug and metabolites.
Half life
Approximately 1.4 to 3.9 hours for dextromethorphan; active metabolite dextrorphan has a longer half-life (10-19 hours).
Absorption
Rapidly absorbed from the gastrointestinal tract.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6, to its active metabolite dextrorphan.
Side Effects
Contraindications
- Hypersensitivity to dextromethorphan or any component of the syrup
- Patients currently taking or who have taken Monoamine Oxidase Inhibitors (MAOIs) within the last 14 days, due to risk of serotonin syndrome
- Patients with productive cough (unless advised by a doctor)
- Asthma and other conditions where cough is beneficial
Drug Interactions
SSRIs and TCAs
Increased risk of serotonin syndrome.
Alcohol and other CNS depressants
May enhance CNS depressant effects like drowsiness and dizziness.
Monoamine Oxidase Inhibitors (MAOIs)
Concurrent use can lead to serotonin syndrome (hyperthermia, rigidity, mental status changes). Avoid use within 14 days of MAOI cessation.
CYP2D6 inhibitors (e.g., fluoxetine, paroxetine, quinidine)
Can increase dextromethorphan plasma concentrations, leading to increased effects and side effects.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, drowsiness, dizziness, ataxia, nystagmus, blurred vision, respiratory depression, and serotonin syndrome (agitation, confusion, tachycardia). Treatment is supportive, and severe cases may require naloxone.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if potential benefit outweighs potential risk, and under medical supervision. Dextromethorphan is excreted into breast milk; use with caution in breastfeeding mothers.
